【摘要】:自發(fā)流產(chǎn)在所有臨床確認的妊娠中,大約占15%,其中孕早期流產(chǎn)約為80%。流產(chǎn)是指妊娠未滿28周、胎兒體重未滿1000g而使妊娠終止,早期流產(chǎn)為發(fā)生在妊娠12周以前者。誘發(fā)自然流產(chǎn)的病因極其復雜,常見原因有:免疫因素、遺傳缺陷、感染性疾病、內(nèi)分泌紊亂、生殖道解剖因素等,這些因素可能同時參與導致流產(chǎn)的發(fā)生。但仍有部分患者發(fā)病誘因不明,有研究顯示這部分自發(fā)流產(chǎn)患者中有40%~80%可能為免疫因素所致。自發(fā)流產(chǎn)不僅對女性患者及家庭帶來眾多困擾,其伴發(fā)生殖器官并發(fā)癥,嚴重者會導致大出血而危及生命。卵泡抑素(Follistatin,FS)是激活素(Activin)結(jié)合蛋白,Activin A是轉(zhuǎn)化生長因子(TGF-β)超家族成員,FS可以中和該家族成員的生物學功能而發(fā)揮其相應(yīng)的作用。有研究發(fā)現(xiàn),妊娠期間FS及Activin A主要來源于母胎界面,即蛻膜和絨毛組織。本課題組前期檢測的小樣本結(jié)果發(fā)現(xiàn)正常孕早期血清FS的表達水平高于自發(fā)流產(chǎn)患者,然而自發(fā)流產(chǎn)患者Activin A水平變化,以及血清FS降低是否能夠真正作為預(yù)測自發(fā)流產(chǎn)的輔助診斷指標,目前尚缺乏深入研究。因此,本次研究首先通過進一步擴大臨床檢測樣本,確定絨毛滋養(yǎng)細胞FS及Activin A的表達與自發(fā)流產(chǎn)之間的關(guān)系;其次,體外培養(yǎng)人絨毛滋養(yǎng)細胞系HTR-8/SVneo細胞,探索FS對該細胞的生物學功能的影響,通過滋養(yǎng)細胞的增殖及分泌活性的分析,進一步揭示FS參與自發(fā)流產(chǎn)的作用機制,對于輔助診斷臨床自發(fā)流產(chǎn),積極進行有效治療,改善患者生活質(zhì)量都具有十分重要的意義。
[Abstract]:Spontaneous abortion accounts for about 15% of all clinically confirmed pregnancies, of which the early stage of pregnancy is about 80%. Abortion refers to the termination of pregnancy within 28 weeks of gestation and less than 1000g of fetal weight. The cause of induced spontaneous abortion is extremely complicated. The common causes are immune factors, genetic defects, infectious diseases, endocrine disorders, anatomical factors of reproductive tract, and so on. These factors may also be involved in the occurrence of miscarriage. Studies have shown that 40% of the patients with spontaneous abortion may be caused by immune factors. Spontaneous abortion not only brings many troubles to female patients and families, but also complicates the complications of reproductive organs. In severe cases, it can cause massive bleeding and endanger life. Follicle inhibitor FollistatinFSis activin Activin-binding protein Activin A is the transforming growth factor TGF- 尾) superfamily member FFS can neutralize the biological function of this family member and play its corresponding role. FS and Activin A during pregnancy were mainly derived from the maternal and fetal interface, i.e. decidual and chorionic villi. The results of small samples in the early stage of pregnancy showed that the expression of FS in the normal early pregnancy was higher than that in the spontaneous abortion. However, the changes of Activin A level in spontaneous abortion patients and whether the decrease of serum FS can really be used as an auxiliary diagnostic index to predict spontaneous abortion is still lack of in-depth study. To determine the relationship between the expression of FS and Activin A in villus trophoblast cells and spontaneous abortion. Secondly, to explore the effect of FS on the biological function of human villus trophoblastic cell line HTR-8/SVneo cells in vitro. Through the analysis of the proliferation and secretory activity of trophoblast, the mechanism of FS involved in spontaneous abortion is further revealed, which is of great significance for the diagnosis of spontaneous abortion, the effective treatment and the improvement of patients' quality of life.
