【摘要】：目的 探討多中心雙胎胎膜早破的臨床特點及妊娠結局,指導臨床對雙胎胎膜早破孕期管理。方法選取2013年1月至2013年12月全國不同城市14家醫(yī)院(中國醫(yī)科大學附屬盛京醫(yī)院,浙江大學醫(yī)學院附屬婦產科醫(yī)院,復旦大學附屬婦產科醫(yī)院,同濟大學附屬第一婦嬰保健院,廣州醫(yī)科大學附屬第三醫(yī)院,山東大學附屬省立醫(yī)院,中山大學附屬第一醫(yī)院,重慶醫(yī)科大學附屬第一醫(yī)院,北京大學第一醫(yī)院,南京大學醫(yī)學院附屬鼓樓醫(yī)院,沈陽市婦嬰醫(yī)院,大連市婦產醫(yī)院,遼陽市第三人民醫(yī)院,本溪市中心醫(yī)院)終止妊娠的3343例雙胎為研究對象,按照是否并發(fā)胎膜早破,分為胎膜早破組468例與非胎膜早破組2875例,比較兩組的年齡、孕產次、人工流產次數、受孕方式、終止妊娠孕周、分娩方式及新生兒窒息情況。結果 雙胎胎膜早破發(fā)生率為13.99%,其中未足月胎膜早破占92.09%。雙胎胎膜早破組早產發(fā)生率84.83%。雙胎胎膜早破組的陰式分娩率、流產率、早產率、新生兒窒息率均高于雙胎非胎膜早破組(29.91%vs.21.01%,9.61%vs.4.24%,84.83%vs.60.18%,12.16%vs.6.30%)。結論 雙胎胎膜早破發(fā)生率高,易導致流產及早產,圍產兒預后差。預防雙胎胎膜早破對改善雙胎妊娠結局至關重要。
[Abstract]:Objective to investigate the clinical characteristics and pregnancy outcome of premature rupture of membranes in multicentric twins. Methods from January 2013 to December 2013, 14 hospitals in different cities (Shengjing Hospital, China Medical University, affiliated Obstetrics and Gynecology Hospital, Zhejiang University Medical College) were selected. The first Hospital of Obstetrics and Gynecology, the first affiliated Hospital of Tongji University, the third affiliated Hospital of Guangzhou Medical University, the Provincial Hospital of Shandong University, the first affiliated Hospital of Sun Yat-sen University, the first affiliated Hospital of Chongqing Medical University, The study included 3343 twins from the first Hospital of Peking University, the Gulou Hospital affiliated to the Medical College of Nanjing University, the Shenyang Women and infants Hospital, the Dalian Obstetrics and Gynecology Hospital, the third people's Hospital of Liaoyang City, and the Central Hospital of Benxi City). According to whether or not the premature rupture of membranes was complicated, they were divided into 468 cases of premature rupture of membranes group and 2875 cases of non-fetal membrane premature rupture group. The age, the number of pregnancy, the times of induced abortion, the mode of conception, and the termination of gestational weeks were compared between the two groups. Results the incidence of premature rupture of membranes in twins was 13.99, of which premature rupture of unmatured membranes was 92.09. The rate of premature delivery in the group of premature rupture of membranes was 84.83. The rate of vaginal delivery, abortion and premature delivery in the group of premature rupture of membranes of twins was 84.83%, and the rate of premature delivery was 84.83% in the group of premature rupture of fetal membrane, and the rate of premature delivery was 84.83% in the group of premature rupture of membranes. The rate of neonatal asphyxia was higher than that of the twin group (29.91vs.21.01and 9.61vs.4.240.84.83vs.60.18v s.60.18v s.6.300.Conclusion the rate of premature rupture of membranes in twins is high, it is easy to cause abortion and premature delivery, and the prognosis of perinatal infants is poor. It is important to prevent premature rupture of membranes for improving the outcome of twin pregnancy.
