【摘要】：目的分析MELF(microcystic elongated and fragmented)浸潤在子宮內(nèi)膜癌診斷及預(yù)后判斷中的臨床價值。方法回顧性分析2011年3月至2013年6月復(fù)旦大學(xué)附屬婦產(chǎn)科醫(yī)院收治的6例子宮內(nèi)膜癌伴有MELF浸潤病理類型患者的病例資料。分析MELF浸潤與子宮內(nèi)膜癌臨床病理類型、分期及預(yù)后的相關(guān)性。結(jié)果患者年齡43~67歲,中位年齡56.5歲,臨床表現(xiàn)以陰道不規(guī)則出血或陰道異常排液為主。1例為子宮內(nèi)膜樣腺癌I級合并透明細(xì)胞癌ⅠB期;1例子宮內(nèi)膜樣腺癌Ⅰ級ⅠB期;3例子宮內(nèi)膜樣腺癌Ⅰ級ⅢC期;1例子宮內(nèi)膜樣腺癌Ⅰ級ⅣB期。其中4例存在淋巴結(jié)轉(zhuǎn)移,5例存在脈管內(nèi)轉(zhuǎn)移。術(shù)后隨訪5~29個月,均未見復(fù)發(fā)及轉(zhuǎn)移。結(jié)論 MELF浸潤多見于較晚期子宮內(nèi)膜樣腺癌,多伴有淋巴結(jié)侵犯。存在MELF浸潤的子宮內(nèi)膜癌應(yīng)尤其注意是否存在脈管累及及淋巴結(jié)轉(zhuǎn)移。
[Abstract]:Objective to analyze the clinical value of MELF(microcystic elongated and fragile infiltration in the diagnosis and prognosis of endometrial carcinoma. Methods six cases of endometrial carcinoma with MELF were retrospectively analyzed from March 2011 to June 2013 in the Department of Obstetrics and Gynecology, Fudan University affiliated Hospital of Obstetrics and Gynecology. Clinical data of patients with infiltrating pathological type. The clinicopathologic types of MELF invasion and endometrial carcinoma were analyzed. Results the age of the patients was 4367 years old and the median age was 56.5 years old. The clinical manifestations were irregular vaginal bleeding or abnormal vaginal effusion in 1 case of endometrial adenocarcinoma grade I with clear cell carcinoma stage 鈪，
