【摘要】：目的探討胎盤因素所致產(chǎn)后出血的原因及有效防治措施。方法對46例胎盤因素所致產(chǎn)后出血的病例資料進(jìn)行回顧性分析。結(jié)果 46例胎盤因素所致產(chǎn)后出血的情況主要表現(xiàn)在胎盤嵌頓、胎盤剝離不全、胎盤粘連、前置胎盤、胎盤植入等,其中多產(chǎn)史3例,人工流產(chǎn)史9例,剖宮產(chǎn)史16例,肌瘤剔除史5例,高齡產(chǎn)婦(≥35歲)7例。46例患者均首選宮縮劑止血,在藥物治療的基礎(chǔ)上仍大出血者聯(lián)合多種"二線"技術(shù),41例救治成功,5例藥物聯(lián)合多種"二線"技術(shù)仍出血不止行子宮切除術(shù)。均救治成功,無1例死亡。結(jié)論近年來胎盤因素所致產(chǎn)后出血比例越來越高,藥物和多種治療技術(shù)聯(lián)合應(yīng)用治療產(chǎn)后出血有效。產(chǎn)后出血重在預(yù)防,降低人工流產(chǎn)率及剖宮產(chǎn)率、盡量減少和預(yù)防胎盤因素勢在必行。
[Abstract]:Objective to investigate the causes and effective prevention and treatment of postpartum hemorrhage caused by placental factors. Methods the data of 46 cases of postpartum hemorrhage caused by placental factors were retrospectively analyzed. Results 46 cases of postpartum hemorrhage caused by placenta factors were analyzed retrospectively. The main manifestation of the situation is placental incarceration. Placental dissection, placental adhesions, placenta previa, placenta accreta, etc. Among them, 3 cases had a history of multiple births, 9 cases had a history of induced abortion, 16 cases had a history of cesarean section, 5 cases had a history of myoma removal. On the basis of drug therapy, 41 cases of patients with massive bleeding and 41 cases of successful treatment with multiple "second line" technique were treated successfully. 5 cases were still bleeding in combination with multiple "second line" technique. All of them were cured successfully. Conclusion the proportion of postpartum hemorrhage caused by placental factors is increasing in recent years. The combination of drugs and treatment techniques is effective in the treatment of postpartum hemorrhage. The focus of postpartum hemorrhage is to prevent, reduce the rate of induced abortion and cesarean section. It is imperative to reduce and prevent placental factors as far as possible.
