【摘要】：目的:研究以自然周期作為子宮內(nèi)膜準(zhǔn)備方式對子宮內(nèi)膜異位癥(EMS)患者凍融胚胎移植(FET)妊娠結(jié)局的影響。方法:回顧性分析EMS患者353個FET周期,按EMS嚴(yán)重程度分組,A組:I~II期,120個周期;B組:III~IV期,233個周期;另將B組中囊腫復(fù)發(fā)的47個周期設(shè)為D組;而將輸卵管因素不孕患者的300個FET周期納入為對照組(C組),比較A、B、C組患者自然周期準(zhǔn)備內(nèi)膜的妊娠結(jié)局。結(jié)果:A、B、C組患者的種植率、活產(chǎn)率、繼續(xù)妊娠率、流產(chǎn)率、妊娠期并發(fā)癥率無統(tǒng)計學(xué)差異(P0.05),且妊娠結(jié)局與EMS的分期無關(guān)。A、B、C組均沒有出生缺陷兒。當(dāng)高質(zhì)量的胚胎移植時,卵巢內(nèi)膜異位囊腫并不影響妊娠結(jié)局。B組較C組低出生體質(zhì)量兒和早產(chǎn)兒的發(fā)生率高。結(jié)論:EMS患者自然周期準(zhǔn)備內(nèi)膜與輸卵管性因素不孕患者有相似的妊娠結(jié)局,且與EMS嚴(yán)重程度無關(guān),妊娠結(jié)局不受內(nèi)膜異位囊腫的影響,是經(jīng)濟(jì)、高效的內(nèi)膜準(zhǔn)備方法。
[Abstract]:Objective: to study the effect of natural cycle as endometrial preparation on pregnancy outcome of frozen and thawed embryo transfer (FET) in patients with endometriosis. Methods: 353 FET cycles in patients with EMS were retrospectively analyzed. According to the severity of EMS, group A was divided into two groups: group A, stage II, group B, group B, stage IV, 233 cycles, and group B, 47 cycles of recurrence of cysts in group B were divided into group D. However, 300 FET cycles of infertile patients with tubal factor were included as control group C to compare the pregnancy outcome of patients with spontaneous cycle preparation of endometrium. Results the implantation rate, live delivery rate, continuous pregnancy rate and abortion rate of the patients in the control group were compared. There was no significant difference in the rate of complications during pregnancy (P 0.05), and the pregnancy outcome was not related to the stage of EMS. There were no birth defects in group A. The incidence of low birth weight infants and premature infants in group B was higher than that in group C. ConclusionThe pregnancy outcome of infertile patients with tubal factors was similar to that of patients with spontaneous cycle preparation of endometrium in group 1: EMS. The pregnancy outcome is not affected by endometriosis cysts and is an economical and efficient method for preparing endometrium.
