【摘要】：研究目的：旨在通過檢測反復(fù)胚胎移植失敗婦女種植窗口期子宮內(nèi)膜COX-2的表達情況。探討其與子宮內(nèi)膜容受性的關(guān)系。為研究改善子宮內(nèi)膜容受性及提高胚胎種植率提供理論依據(jù)。 方法：選取2013年8月-12月在廣西醫(yī)科大學(xué)第一附屬醫(yī)院生殖中心就診的不孕婦女共44例。常規(guī)接受3次或以上ET/FET均未獲得臨床妊娠患者16例分為A組；同期首次ET未獲得妊娠患者15例分為B組；同期首次ET獲得臨床妊娠患者13例分為C組。應(yīng)用熒光定量PCR技術(shù)檢測上述不孕患者種植窗口期子宮內(nèi)膜COX-2的mRNA表達情況，，探討其與子宮內(nèi)膜容受性的關(guān)系。 結(jié)果：1.A.B.C三組患者一般資料的比較，如年齡、不孕年限、體重指數(shù)、不孕類型、不孕因素，受精方式，差異均無統(tǒng)計學(xué)意義（P＞0.05)。 2. A.B.C三組患者卵巢COH的比較及移植日子宮內(nèi)膜厚度比較：基礎(chǔ)FSH；Gn天數(shù)、Gn量，HCG日E2、P，獲卵數(shù)、胚胎移植日子宮內(nèi)膜厚度的比較差異無統(tǒng)計學(xué)意義（P＞0.05)。 3. A.B.C三組患者種植窗口期子宮內(nèi)膜COX-2mRNA表達情況：3次或以上ET/FET均未獲得臨床妊娠患者（A組）、同期首次ET未獲得妊娠患者（B組）、同期首次ET獲得臨床妊娠患者（C組）子宮內(nèi)膜COX-2的mRNA平均相對含量分別為：A組：0.0263±0.0257，B組：0.0522±0.0309，C組：0.0598±0.0326。三組間差異有統(tǒng)計學(xué)意義(P＜0.05)。首次ET獲得妊娠患者種植窗期子宮內(nèi)膜的COX-2的mRNA的表達（C組）是反復(fù)移植失敗患者（A組）的2.3倍，差異有統(tǒng)計學(xué)意義(P＜0.05)。首次ET未獲得妊娠患者種植窗期子宮內(nèi)膜的COX-2的mRNA的表達（B組）是反復(fù)移植失敗患者（A組）2倍，差異有統(tǒng)計學(xué)意義(P＜0.05)。首次ET未獲得妊娠組(B組)與首次ET獲得妊娠組（C組）比較差異無統(tǒng)計學(xué)意義（P＞0.05）。 結(jié)論：反復(fù)胚胎移植失敗患者子宮內(nèi)膜COX-2的mRNA平均相對表達量低于首次ET獲得臨床妊娠的患者。提示種植窗期子宮內(nèi)膜COX-2的表達降低或缺乏，可能導(dǎo)致子宮內(nèi)膜容受性下降，影響胚胎種植。
[Abstract]:Objective: to investigate the expression of endometrial COX-2 in women with repeated embryo transfer failure during implantation window, and to explore the relationship between endometrial receptivity and endometrial receptivity. Planting rate provides theoretical basis. Methods: a total of 44 infertile women admitted to the Reproductive Center of the first affiliated Hospital of Guangxi Medical University from August 2013 to December were selected. At the same time, 15 cases of pregnancy without et were divided into group B and 13 cases of clinical pregnancy were divided into group C. the expression of COX-2 in endometrium of the infertile patients during implantation window was detected by fluorescence quantitative PCR. To investigate the relationship between endometrial receptivity and endometrial receptivity. Results there was no significant difference in general data, such as age, years of infertility, body mass index, type of infertility, factors of infertility, and fertilization mode among the three groups (P > 0.05). 2. Comparison of ovarian COH and endometrial thickness on transplantation day in A.B.C group: there was no significant difference in endometrial thickness on embryo transfer day (P > 0.05). 3. Expression of COX-2mRNA in endometrium of three groups of A.B.C patients during implantation window period: no clinical pregnancy group (group A) was obtained for 3 times or more of ET/FET, group B was not obtained for the first time et, and group C (group C) for the first time. The average relative mRNA content of endometrial COX-2 was: 1: a group: 0.0263 鹵0.0257%, group B: 0.0522 鹵0.0309%, group C: 0.0598 鹵0.0326.The difference between the three groups was statistically significant (P < 0.05). The expression of COX-2 in the implantation window of pregnancy patients was obtained by et for the first time (group C) was a failure of repeated transplantation. 2. 3 times as much as group A. The difference was significant (P < 0.05). The mRNA expression of COX-2 in the endometrium during implantation window stage was 2 times higher in group B than in group A, which was failed in repeated transplantation for the first time (P < 0.05). The difference was statistically significant (P < 0.05). There was no significant difference between group B (group B) and group C (group C) (P > 0.05). Conclusion: the average relative expression of mRNA in endometrial COX-2 in patients with failed repeated embryo transfer is lower than that in patients with clinical pregnancy obtained by et for the first time. It is suggested that the decrease or lack of COX-2 expression in endometrium during implantation window may lead to the decrease of endometrial receptivity. Affect embryo implantation.
