【摘要】：目的探討妊娠中期正常糖耐量(NGT)孕婦糖化血清白蛋白(GA)的正常參考值范圍及其篩查GDM的臨床意義。方法選取24~28周孕婦189名,其中,GDM(GDM組)患者63例,正常對(duì)照者(NGT組)126名。測(cè)定GA、FPG等相關(guān)指標(biāo),分析孕中期GA正常參考值水平,同時(shí)應(yīng)用受試者工作特征(ROC)曲線得出篩查GDM最佳切點(diǎn)及其敏感性、特異性。結(jié)果 (1)NGT組孕中期GA的正常參考值范圍為11.1%~15.3%;(2)若GA為15.2%、FPG為4.35mmol/L,診斷GDM的敏感性分別為98.4%、82.5%,特異性分別為96.8%、66.7%。(3)孕24~28周FPG≥4.35mmol/L或GA≥15.2%者需行75g OGTT,這將有助于減少43.8%的孕婦行75g OGTT。結(jié)論孕24~28周同時(shí)檢測(cè)FPG及GA,可提高GDM篩查的準(zhǔn)確性及敏感性。若FPG≥4.35mmol/L或GA≥15.2%的孕婦行75g OGTT,則建議將11.1%~15.3%作為妊娠中期GA的正常參考值范圍。
[Abstract]:Objective to investigate the normal reference range of serum albumin (GAGA) in pregnant women with normal glucose tolerance (NGT) in the second trimester of pregnancy and its clinical significance in screening GDM. The normal reference value of GA in the second trimester of pregnancy was analyzed, and the optimum point of GDM screening and its sensitivity were obtained by using the operating characteristics of the subjects. Results the normal reference value of GA in NGT group was 11.1mmol / L and 15.3mmol / L respectively.
