【摘要】：目的探討超聲引導(dǎo)下將中期多胎妊娠減至雙胎的安全性,以檢驗(yàn)該方法的可行性。資料與方法 43例接受胚胎移植的孕婦按照是否行減胎術(shù)分為:減胎組16例,于妊娠中期在超聲引導(dǎo)下行減胎術(shù)、將多胎妊娠減至雙胎;非減胎組27例,為雙胎妊娠孕婦。比較兩組孕婦妊娠結(jié)局的差異。結(jié)果減胎組中,1例疑似宮內(nèi)感染流產(chǎn),1例已分娩者于減胎后2 d內(nèi)保留胎兒之一死亡,其余14例已分娩8例,6例仍在妊娠中;截止至2014年6月11日共獲得17名新生兒,無(wú)新生兒死亡及畸形。非減胎組中,1例自然流產(chǎn),2例已分娩者出現(xiàn)雙胎之一停育,其余24例全部分娩;共獲得50名新生兒,新生兒畸形3名,無(wú)新生兒死亡。兩組在出生孕周、新生兒體重、母嬰并發(fā)癥等方面差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論將多胎減至雙胎改善了妊娠結(jié)局,與未減胎雙胎相比,兩者妊娠結(jié)局無(wú)顯著差異,可以認(rèn)為妊娠中期將多胎減至雙胎安全可行。
[Abstract]:Objective to investigate the safety of ultrasound guided multiple pregnancy reduction to twin pregnancy, and to test the feasibility of this method. Materials and methods 43 pregnant women undergoing embryo transfer were divided into two groups according to whether or not to undergo fetal reduction. In the second trimester of pregnancy, the multiple pregnancy was reduced to twins under the guidance of ultrasound, and 27 cases in the non-reduction group. Results one case of suspected intrauterine infection abortion and one case of delivery had died within 2 days after pregnancy reduction, and the other 14 cases had delivered 8 cases or 6 cases were still in pregnancy. As of June 11th 2014, a total of 17 newborns had been obtained, with no neonatal death or malformation. In the non-reduced group, 1 case of spontaneous abortion and 2 cases of spontaneous abortion had stopped giving birth, while the remaining 24 cases had all given birth; a total of 50 newborns were born. There were 3 cases of neonatal malformation and no neonatal death. There was no significant difference between the two groups in gestational week, weight of newborn and complications of mother and child. Conclusion reducing the number of multiple twins to twins improves the outcome of pregnancy, and compared with that of unreduced twins, there is no significant difference between the two groups. There is no significant difference in pregnancy outcome between the two groups, so it is safe and feasible to reduce the number of multiple pregnancies to twins in the second trimester of pregnancy.
