【摘要】：目的:探討在控制性卵巢刺激(COS)周期中晚卵泡期孕酮(P)水平變化趨勢對(duì)體外受精-胚胎移植(IVF-ET)結(jié)局的影響。方法:回顧分析2013年1月至2014年6月在我中心行IVF-ET長方案助孕治療的676個(gè)周期。按促排卵中晚卵泡期起始日(主導(dǎo)卵泡直徑達(dá)13mm日)與人絨毛膜促性腺激素(HCG)注射日孕酮水平變化,分為升高趨勢組和降低趨勢組;按HCG日孕酮水平是否1.0ng/ml將升高趨勢組和降低趨勢組中新鮮移植周期分別分為A組(P≤1ng/ml)、B組(P1ng/ml)和C組(P≤1ng/ml)、D組(P1ng/ml)。結(jié)果:(1)升高趨勢組與降低趨勢組的卵裂率、優(yōu)質(zhì)胚胎率比較,差異有統(tǒng)計(jì)學(xué)意義(99.65%vs 99.00%,70.57%vs 63.09%;P0.05);(2)A組與B組的臨床妊娠率、活產(chǎn)率比較,差異有統(tǒng)計(jì)學(xué)意義(56%vs 43.00%;50%vs 37.76%,P0.05);C組與D組的臨床妊娠率、活產(chǎn)率比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:IVF-ET長方案周期中,COS過程中孕酮變化呈升高趨勢的優(yōu)質(zhì)胚胎率較降低趨勢高;新鮮移植周期,P呈升高趨勢時(shí),HCG日P1.0ng/ml,活產(chǎn)率下降;呈降低趨勢時(shí),HCG日P水平不影響活產(chǎn)率。
[Abstract]:Objective: To explore the effects of controlled ovarian stimulation (COS) cycle late follicular phase progesterone (P) level change trend of in vitro fertilization and embryo transfer (IVF-ET) outcome. Methods: a retrospective analysis of 676 cycles from January 2013 to June 2014 in our center for IVF-ET protocol assisted reproductive treatment. According to the late follicular ovulation period starting Japan (the dominant follicle diameter up to 13mm days) and human chorionic gonadotropin (HCG) injection on the changes of progesterone level, divided into group increased and decreased HCG level of progesterone group; according to whether 1.0ng/ml will increase the tendency of decreasing group and the group of fresh transfer cycles were divided into A group (P = 1ng/ml), B group (P1ng/ml) and C group (P = 1ng/ml), D group (P1ng/ml). Results: (1) increased with decreasing trend group group cleavage rate, high-quality embryo rate comparison, the difference was statistically significant (99.65%vs 99%, 70.57%vs 63.09%; P0.05); (2) clinical pregnancy group A with the B group Pregnancy rate, live birth rate comparison, the difference was statistically significant (56%vs 43%; 50%vs 37.76%, P0.05); C group and D group, the clinical pregnancy rate, live birth rate comparison, the difference was not statistically significant (P0.05). Conclusion: IVF-ET long project cycle, changes in the process of COS is progesterone trend rate of high quality embryos increased a decreasing trend is high; the fresh transplantation period, P increased, HCG, P1.0ng/ml, the live birth rate decreased; decreased when HCG P level does not affect the live birth rate.
