【摘要】：目的探討經(jīng)陰網(wǎng)片全盆重建手術(shù)治療盆腔器官脫垂的療效及并發(fā)癥,評估該手術(shù)的有效性和安全性。方法回顧性分析2007年1月至2014年6月在廣州醫(yī)科大學(xué)附屬第一醫(yī)院因盆腔器官脫垂行經(jīng)陰網(wǎng)片全盆重建術(shù)110例患者的臨床資料,比較手術(shù)前后盆腔器官脫垂定量分期法(POP-Q)各指示點(diǎn)位置,評價(jià)總體客觀治愈率及復(fù)發(fā)率。采用盆底功能障礙性疾病癥狀問卷簡表(PFDI-20)等相關(guān)問卷評分評價(jià)主觀治愈率以及術(shù)后尿失禁相關(guān)癥狀改善情況。結(jié)果 110例患者中失訪12例(12/110,10.91%),其中1例因心血管疾病術(shù)后1年余死亡,隨訪98例(98/110,89.09%)。隨訪時(shí)間6~90個(gè)月,中位隨訪時(shí)間:48個(gè)月。110例患者年齡(65.30±8.00)歲;絕經(jīng)時(shí)間(14.01±9.08)年;采用POP-Q分期評價(jià),術(shù)后15例(15/98,15.31%)出現(xiàn)復(fù)發(fā),其中11例POP-Q分期Ⅱ期,無主觀癥狀不需再次手術(shù),4例陰道頂端復(fù)發(fā)(POP-QⅢ期),需再次手術(shù)治療。術(shù)后主觀治愈率95.92%(94/98),客觀治愈率84.69%(83/98);網(wǎng)片攣縮4例(4/98,4.08%),網(wǎng)片外露11例(11/98,11.22%)。術(shù)后PFDI-20評分與術(shù)前比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后性生活13例,其中性生活質(zhì)量5例滿意,6例患者較術(shù)前差,2例配偶出現(xiàn)性交疼痛;患者總體術(shù)后滿意度評分滿意率95.91%(94/98)。結(jié)論經(jīng)陰網(wǎng)片全盆重建術(shù)用于治療重度盆腔器官脫垂主、客觀治愈率高,不僅能達(dá)到較好的解剖學(xué)復(fù)位,對患者的盆腔器官脫垂相關(guān)癥狀有明顯改善,術(shù)后滿意度高且長期療效穩(wěn)定,但對性生活質(zhì)量無明顯改善。
[Abstract]:Objective to investigate the curative effect and complications of total pelvic reconstruction with transvaginal mesh in the treatment of pelvic organ prolapse. To evaluate the efficacy and safety of the operation methods from January 2007 to June 2014 in the first affiliated Hospital of Guangzhou Medical University performed total pelvic reconstruction through negative mesh for pelvic organ prolapse: a retrospective analysis of 110 cases.
