葡萄糖調(diào)節(jié)蛋白78在宮頸鱗癌中的表達(dá)及其臨床意義

【摘要】:目的:探討葡萄糖調(diào)節(jié)蛋白78(glucose-regulated protein78,GRP78)在宮頸鱗癌中的表達(dá)以及與宮頸鱗癌發(fā)生、發(fā)展的關(guān)系。 方法:收集四川大學(xué)華西臨床醫(yī)學(xué)院2011年6月至2013年6月間經(jīng)手術(shù)切除且經(jīng)病理證實的宮頸鱗癌標(biāo)本98例、宮頸上皮內(nèi)瘤變(cervical intraepithelial neoplasis,CIN)30例和正常宮頸組織30例。采用免疫組化SP法檢測GRP78在宮頸鱗癌、CIN和正常宮頸組織中的表達(dá)情況。并用統(tǒng)計學(xué)軟件SPSS13.0分析宮頸鱗癌組織中GRP78的表達(dá)與其臨床病理參數(shù)之間的關(guān)系。 結(jié)果:宮頸鱗癌組織中GRP78陽性細(xì)胞的比例明顯高于CIN和正常宮頸組織(P0.05)。98例宮頸鱗癌組織中GRP78蛋白陽性表達(dá)率為80.6%(79/98),30例CIN組織中GRP78陽性表達(dá)率為50%(15/30),30例正常宮頸組織中GRP78陽性表達(dá)率為43.3%(13/30);正常宮頸組與CIN組之間差異無統(tǒng)計學(xué)意義(P=0.829),CIN組與宮頸鱗癌組之間差異有統(tǒng)計學(xué)意義(P=0.004),正常宮頸組與宮頸鱗癌組之間差異有統(tǒng)計學(xué)意義(P0.001)。無淋巴轉(zhuǎn)移的宮頸鱗癌組織中GRP78的陽性表達(dá)率為45.3%,伴有淋巴轉(zhuǎn)移的宮頸鱗癌中GRP78的陽性表達(dá)率為82.3%。結(jié)果顯示伴有淋巴結(jié)轉(zhuǎn)移的癌組織中GRP78的表達(dá)明顯高于無淋巴結(jié)轉(zhuǎn)移的癌組織,差別具有統(tǒng)計學(xué)意義(P<0.05),提示GRP78表達(dá)增加與淋巴結(jié)轉(zhuǎn)移相關(guān)。伴HR-HPV感染的宮頸鱗癌組織中GRP78的陽性表達(dá)率為78.5%,不伴HR-HPV感染的宮頸鱗癌組織中GRP78的陽性表達(dá)率為47.4%,兩組比較,差別有統(tǒng)計學(xué)意義(P<0.05),,表明GRP78的過表達(dá)與宮頸鱗癌組織是否伴HR-HPV感染有關(guān),同時GRP78的表達(dá)與宮頸鱗癌患者的年齡、臨床分期、病理分級無明顯相關(guān)性(P>0.05)。 結(jié)論:GRP78在宮頸鱗癌組織細(xì)胞中呈現(xiàn)高表達(dá),而在CIN及正常宮頸組織中呈低表達(dá)。GRP78的表達(dá)與宮頸鱗癌的淋巴結(jié)轉(zhuǎn)移及HR-HPV感染有關(guān),而與患者的年齡、臨床分期、病理分級無關(guān)。本研究表明GRP78可能參與了宮頸鱗癌發(fā)生、發(fā)展的過程,可能與腫瘤的惡性細(xì)胞增生有關(guān)。
[Abstract]:Objective: to investigate the expression of glucose-regulated glucose-regulated GRP78 in cervical squamous cell carcinoma (SCC) and its association with cervical squamous cell carcinoma (SCC). The relationship between development Methods: 98 cases of cervical squamous cell carcinoma (SCC) surgically resected and confirmed by pathology from June 2011 to June 2013 were collected from West China College of Clinical Medicine of Sichuan University. Cervical intraepithelial neoplasia with cervical intraepithelial neoplasis. Immunohistochemical SP method was used to detect GRP78 in cervical squamous cell carcinoma (SCC). The relationship between the expression of GRP78 and clinicopathological parameters in squamous cell carcinoma of cervix was analyzed by SPSS13.0. Results: the proportion of GRP78 positive cells in cervical squamous cell carcinoma was significantly higher than that in CIN and normal cervical tissue (P0.05). The positive expression rate of GRP78 protein in 98 cases of cervical squamous cell carcinoma was 80.679 / 98). The positive expression rate of GRP78 in 30 cases of CIN was 50%. The positive rate of GRP78 in 30 cases of normal cervix was 43.3%. There was no significant difference between normal cervix group and CIN group. There was a significant difference between normal cervical cancer and cervical squamous cell carcinoma. The positive rate of GRP78 in cervical squamous cell carcinoma without lymphatic metastasis was 45.3%. The positive rate of GRP78 expression in cervical squamous cell carcinoma with lymphatic metastasis was 82.3%. The results showed that the expression of GRP78 was significantly higher in carcinoma with lymph node metastasis than in non-lymph node metastasis. The difference was statistically significant (P < 0.05). It was suggested that the increase of GRP78 expression was related to lymph node metastasis, and the positive rate of GRP78 expression in cervical squamous cell carcinoma with HR-HPV infection was 78.5%. The positive expression rate of GRP78 in cervical squamous cell carcinoma without HR-HPV infection was 47.4% (P < 0.05). The results showed that the overexpression of GRP78 was related to whether the cervical squamous cell carcinoma tissues were associated with HR-HPV infection, and the expression of GRP78 was related to the age and clinical stage of patients with cervical squamous cell carcinoma. There was no significant correlation between pathological grade and pathological grade (P > 0.05). ConclusionTwo one GRP78 is highly expressed in squamous cell carcinoma of cervix. However, the low expression of .GRP78 in CIN and normal cervical tissues was related to lymph node metastasis and HR-HPV infection of squamous cell carcinoma of the cervix, but it was related to the age and clinical stage of the patients. This study suggests that GRP78 may be involved in the occurrence and development of squamous cell carcinoma of the cervix and may be related to the malignant cell proliferation of the tumor.
