【摘要】:目的:探討應(yīng)用時(shí)差成像(TLI)技術(shù)基于胚胎卵裂模式及時(shí)間參數(shù)進(jìn)行胚胎挑選,對(duì)分裂期胚胎單胚胎移植(SET)妊娠結(jié)局的影響。方法:1將206個(gè)常規(guī)IVF周期患者胚胎行TLI動(dòng)態(tài)觀察,對(duì)其2 349枚非移植胚胎進(jìn)行囊胚培養(yǎng),回顧性分析胚胎發(fā)育的卵裂模式和時(shí)間參數(shù),以囊胚質(zhì)量為結(jié)局進(jìn)行Logistic回歸分析,建立優(yōu)質(zhì)胚胎篩選標(biāo)準(zhǔn)。2應(yīng)用建立的優(yōu)質(zhì)胚胎篩選標(biāo)準(zhǔn)開展分裂期胚胎SET的前瞻性研究,用TLI觀察88例常規(guī)IVF周期患者的胚胎發(fā)育情況,挑選優(yōu)質(zhì)胚胎進(jìn)行SET,觀察妊娠結(jié)局。結(jié)果:1分裂方式為正常卵裂、不均卵裂、非軸性卵裂的胚胎具有較高的優(yōu)質(zhì)囊胚形成率(分別為57.09%,44.44%及40.91%),時(shí)間參數(shù)從3-細(xì)胞分裂到4-細(xì)胞持續(xù)的時(shí)間(s2)(≤0.50 h)和發(fā)育至5-細(xì)胞的時(shí)間(t5)(≤48.93 h)對(duì)優(yōu)質(zhì)囊胚形成最具預(yù)測價(jià)值。2由此建立基于胚胎卵裂模式與時(shí)間參數(shù)的分層篩選模式,將其應(yīng)用于分裂期胚胎SET,符合篩選條件的SET組臨床妊娠率為73.24%(52/71)。結(jié)論:應(yīng)用TLI,基于胚胎卵裂模式和時(shí)間參數(shù)建立了分層篩選模式,所挑選的優(yōu)質(zhì)胚胎可顯著改善分裂期胚胎SET的妊娠結(jié)局。
[Abstract]:Objective: to investigate the application of time-difference imaging (TLI) technique in embryo selection based on embryo cleavage model and time parameters. Methods one hundred and twenty six embryos in normal IVF cycle were observed dynamically by TLI. The blastocysts of 2 349 non-transplanted embryos were cultured. The cleavage pattern and time parameters of embryo development were analyzed retrospectively. The Logistic regression analysis was carried out with the result of blastocyst quality. A prospective study on SET of mitotic embryos was carried out by using the established criteria of high quality embryo screening. 88 cases of normal IVF cycle patients were observed by TLI.
