【摘要】:目的:探討促性腺激素釋放激素拮抗劑(GnRH antagonist)方案超促排卵過程中推遲人絨毛膜促性腺激素(HCG)扳機時機對胚胎質量及妊娠率等的影響。方法:回顧性分析2015年1月至12月在我院接受體外受精-胚胎移植(IVF-ET)助孕的不孕癥患者183例,均采用GnRH拮抗劑促排方案,于月經周期第2天啟用促性腺激素(Gn),當有卵泡平均直徑達到14mm,加用GnRH拮抗劑。按照傳統(tǒng)HCG扳機時機(有3個≥17mm卵泡)與推遲1天扳機分為2組:早期HCG組(149例)和晚期HCG組(34例),比較兩組數(shù)據(jù)。結果:HCG扳機日,晚期HCG組≥15mm的卵泡數(shù)明顯多于早期HCG組(P=0.026)。晚期HCG組Gn使用天數(shù)及Gn使用總量均明顯高于早期HCG組(P=0.000,P=0.012)。妊娠結局方面,晚期HCG組較早期HCG組具有更高的妊娠率(76.00%vs 50.45%,P=0.020)。兩組受精率、繼續(xù)妊娠率、流產率、異位妊娠率均無顯著差異(P0.05)。結論:GnRH拮抗劑促排方案中,適當推遲HCG扳機時間不影響胚胎質量和妊娠率,可以推行。
[Abstract]:Objective: to investigate the delay of human chorionic gonadotropin (HCG) during hyperstimulation of ovulation by gonadotropin releasing hormone antagonist (GnRH antagonistist). Effects of trigger timing on embryo quality and pregnancy rate. Methods: IVF-ETs received in our hospital from January 2015 to December were retrospectively analyzed. 183 cases of infertility assisted pregnancy.
