【摘要】：背景:合成補(bǔ)片是重要的盆底組織替代物,能夠替代薄弱受損的盆底筋膜組織,己被廣泛應(yīng)用于盆底薄弱組織的修補(bǔ)重建中。目的:觀察補(bǔ)片在改良盆底重建修復(fù)盆腔臟器脫垂的療效及并發(fā)癥,探討其安全性及有效性。方法:納入45例女性盆腔臟器脫垂患者,根據(jù)患者意愿分兩組治療,試驗(yàn)組25例經(jīng)陰道植入聚丙烯補(bǔ)片進(jìn)行改良盆底重建修復(fù),對(duì)照組20例采用傳統(tǒng)經(jīng)陰道全子宮切除聯(lián)合陰道壁修補(bǔ)治療,記錄兩組圍手術(shù)期情況,隨訪評(píng)價(jià)兩組子宮脫垂分期、并發(fā)癥、盆底功能及盆底不適情況,以及客觀治愈率。結(jié)果與結(jié)論:與對(duì)照組比較,試驗(yàn)組手術(shù)時(shí)間短、出血量小、感染輕、恢復(fù)快(P均0.05)。隨訪6個(gè)月時(shí),試驗(yàn)組子宮脫垂分期評(píng)分高于對(duì)照組;隨訪12個(gè)月時(shí),試驗(yàn)組并發(fā)癥發(fā)生率、盆底功能影響評(píng)分及盆底不適情況評(píng)分均低于對(duì)照組(P0.05),客觀治愈率高于對(duì)照組(P0.05)。表明采用聚丙烯補(bǔ)片進(jìn)行盆底重建修復(fù)盆腔臟器脫垂具有手術(shù)時(shí)間短、出血量小、恢復(fù)快、裸露率低等特點(diǎn),同時(shí)能顯著提高盆底功能,改善盆底不適情況,降低術(shù)后并發(fā)癥的發(fā)生,提高客觀治愈率,短期隨訪安全有效。
[Abstract]:Background: synthetic patches are important pelvic floor tissue substitutes for weak damaged pelvic floor fascia. It has been widely used in the repair and reconstruction of weak pelvic floor tissue. Objective: to observe the curative effect and complications of the modified pelvic floor reconstruction in the repair of pelvic organ prolapse. Methods: 45 female patients with pelvic organ prolapse were divided into two groups according to the wishes of the patients and 25 patients in the experimental group were treated with modified pelvic floor reconstruction. 20 cases in the control group were treated with traditional transvaginal hysterectomy combined with vaginal wall repair. The perioperative period of the two groups was recorded. The stage of uterine prolapse, complications, pelvic floor function and pelvic floor discomfort were evaluated. Results and conclusion: compared with the control group, the experimental group had shorter operation time, less bleeding, less infection and better recovery (P < 0.05). The stage score of uterine prolapse in the test group was higher than that in the control group. At 12 months follow-up, the incidence of complications, pelvic floor function impact score and pelvic floor discomfort score in the trial group were lower than those in the control group (P 0.05). The objective cure rate was higher than that of the control group (P0.05A), which indicated that the pelvic floor reconstruction and repair of pelvic viscera prolapse with polypropylene patch had the characteristics of short operation time, small amount of bleeding, quick recovery and low rate of nudity. At the same time, it can significantly improve pelvic floor function, improve pelvic floor discomfort, reduce postoperative complications, improve the objective cure rate, and short-term follow-up is safe and effective.
