【摘要】：目的探討高頻電刀在腹腔鏡下宮頸癌手術(shù)中的應(yīng)用價值。方法河北醫(yī)科大學第四醫(yī)院婦科于2006年11月至2015年8月應(yīng)用高頻電刀和超聲刀行腹腔鏡下根治性子宮切除+盆腔淋巴結(jié)切除及(或)腹主動脈旁淋巴結(jié)切除共530例,對比分析兩組各項手術(shù)指標以及并發(fā)癥之間的差異。結(jié)果兩種手術(shù)器械均能順利完成手術(shù),兩組術(shù)中淋巴結(jié)切除時間、淋巴結(jié)切除個數(shù)、根治性子宮切除時間、術(shù)中出血量、術(shù)后肛門排氣時間、住院天數(shù)、術(shù)后拔除尿管時間、術(shù)后拔除引流管時間等差異均無統(tǒng)計學意義(P0.05)。兩組血管損傷、輸尿管損傷、膀胱損傷、腸道損傷、術(shù)后腸梗阻、淋巴囊腫等并發(fā)癥比較差異均無統(tǒng)計學意義(P0.05)。結(jié)論應(yīng)用高頻電刀行腹腔鏡下根治性子宮切除+盆腔淋巴結(jié)切除及(或)腹主動脈旁淋巴結(jié)切除能達到同超聲刀相同的手術(shù)效果,具有一定臨床應(yīng)用價值。
[Abstract]:Objective to evaluate the value of high frequency electric knife in laparoscopic surgery for cervical cancer. Methods from November 2006 to August 2015, gynecology department of 4th Hospital of Hebei Medical University used high frequency electric knife and ultrasonic knife to perform laparoscopy. Lower radical hysterectomy. Pelvic lymphadenectomy and (. Or abdominal para-aortic lymph node resection in 530 cases. Results the two kinds of surgical instruments were able to complete the operation successfully. The time of lymph node resection, the number of lymph node resection, the time of radical hysterectomy in the two groups were compared and analyzed. There were no significant differences in blood loss, postoperative anal exhaust time, hospital stay, time of extubation of urine tube and time of extubation of drainage tube between the two groups (P 0.05). Vascular injury and ureteral injury were found in the two groups. Bladder injury, intestinal injury, postoperative intestinal obstruction. There was no significant difference in lymphocyst complications (P 0.05). Conclusion Laparoscopic radical hysterectomy and / or pelvic lymphadenectomy are performed with high frequency electric knife. Lymph node resection of abdominal aorta can achieve the same effect as ultrasound knife. It has certain clinical application value.
