枸櫞酸氯米芬聯(lián)合二甲雙胍對多囊卵巢綜合征患者排卵效果及血管活性因子的影響

【摘要】:目的探討枸櫞酸氯米芬聯(lián)合二甲雙胍對多囊卵巢綜合征(PCOS)患者排卵效果及血管活性因子的影響。方法將2011年7月至2014年7月深圳市寶安區(qū)人民醫(yī)院收治的PCOS患者70例按照隨機(jī)數(shù)字表法分為聯(lián)合組和對照組,各35例。對照組口服枸櫞酸氯米芬膠囊,1粒/d,共服用5 d;在對照組的基礎(chǔ)上,聯(lián)合組餐后0.5 h加用口服二甲雙胍,0.5 g/次,3次/d,共服用4周。比較兩組治療后排卵情況,并檢測兩組治療前和治療1個(gè)月后性激素、血脂及血管活性因子水平。結(jié)果對照組治療后睪酮(T)、胰島素(INS)顯著降低,聯(lián)合組治療后黃體生成素(LH)、LH/卵泡雌激素(FSH)、T、INS均較治療前顯著降低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);其中治療后聯(lián)合組LH、LH/FSH、T、INS均顯著低于對照組[(5.79±0.73)U/L比(9.98±1.47)U/L,(1.21±0.13)比(2.00±0.23),(1.41±0.20)nmol/L比(1.75±0.26)nmol/L,(19.87±6.72)U/L比(27.39±8.26)U/L],差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療后兩組患者總膽固醇(TC)、三酰甘油(TG)、低密度脂蛋白膽固醇(LDL-C)均較治療前顯著降低,高密度脂蛋白膽固醇(HDL-C)較治療前顯著升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);治療后聯(lián)合組TC、TG、LDL-C顯著低于對照組[(3.45±0.31)mmol/L比(3.95±0.47)mmol/L,(1.12±0.17)mmol/L比(1.43±0.21)mmol/L,(2.28±0.42)mmol/L比(2.49±0.46)mmol/L],HDL-C顯著高于對照組[(1.98±0.51)mmol/L比(1.62±0.24)mmol/L],差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療后兩組患者血清血管緊張素Ⅱ(AT-Ⅱ)、血管內(nèi)皮生長因子(VEGF)均較治療前顯著降低,聯(lián)合組血清AT-Ⅱ、VEGF水平顯著低于對照組[(1.21±0.16)ng/L比(1.43±0.20)ng/L,(0.23±0.02)ng/L比(0.29±0.04)ng/L],差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論二甲雙胍聯(lián)合枸櫞酸氯米芬能夠調(diào)節(jié)PCOS患者血清性激素、血脂及血管活性因子水平,改善卵巢排卵功能,值得臨床推廣。
[Abstract]:Objective to investigate the clomiphene citrate combined with metformin on polycystic ovary syndrome (PCOS) patients and vasoactive factors affect ovulation effect. Methods from July 2011 to July 2014 in Baoan District people's Hospital of Shenzhen city from 70 PCOS patients were randomly divided into combined group and control group, each of 35 cases. Control group was treated with Clomifene Citrate Capsules, 1 /d, taking a total of 5 d; on the basis of the control group, combined group 0.5 h after meal with oral metformin, 0.5 g/, 3 /d, taking a total of 4 weeks. Ovulation after treatment between the two groups, and two groups were detected before treatment and after treatment for 1 months, sex hormone, blood lipid and the level of vasoactive factors.
