【摘要】：目的探討孕酮誘導的封閉因子(PIBF)與妊娠早期先兆流產(chǎn)的關系以及地屈孕酮的胚胎保護作用。方法黃體功能不全的先兆流產(chǎn)患者42例為實驗組,給予地屈孕酮治療2周;同期健康早期妊娠婦女40例為對照組。采用酶聯(lián)免疫吸附方法(ELISA)檢測兩組孕婦血清PIBF、孕酮濃度并進行分析。結果兩組孕婦的年齡、孕次和治療前后妊娠時間比較差異無統(tǒng)計學意義(P0.05)。治療前,實驗組血清孕酮及PIBF水平低于對照組(P0.05);治療后兩者均呈上升趨勢(P0.05),PIBF達到對照組水平(P0.05),孕酮低于對照組(P0.05)。對照組觀察后血清PIBF水平呈明顯上升趨勢,孕酮小幅上升(P0.05)。觀察前,對照組血清孕酮與PIBF之間呈正相關(P0.05);觀察后,兩變量間不存在相關性;無論在治療前還是治療后,實驗組兩變量間均不存在相關性(P0.05)。結論妊娠早期血清低水平的PIBF是引起先兆流產(chǎn)的因素之一。地屈孕酮的胚胎保護作用可能是通過上調血清PIBF表達水平來實現(xiàn)的。動態(tài)檢測孕婦血清PIBF水平有助于先兆流產(chǎn)的臨床診斷和地屈孕酮的療效評估。
[Abstract]:Objective to investigate the relationship between progesterone-induced blocking factor (PIBF) and threatened abortion in early pregnancy and the protective effect of Diproprogesterone on embryo. Methods 42 patients with threatened abortion with luteal dysfunction were treated as experimental group. The patients were treated with diproprogesterone for 2 weeks. The serum PIBFand progesterone concentrations of 40 healthy early pregnant women in the same period were measured and analyzed by Elisa. Results the age of the two groups was analyzed. There was no significant difference between pregnant time and pregnancy time before and after treatment. Before treatment, the levels of serum progesterone and PIBF in the experimental group were lower than those in the control group (P 0.05). After treatment, both of them showed an increasing trend. P0.05 PIBF reached the level of control group (P0.05). Progesterone was lower than the control group (P 0.05). After observation, the serum PIBF level in the control group showed an obvious upward trend, and the progesterone increased slightly (P 0.05). There was a positive correlation between serum progesterone and PIBF in the control group (P 0.05). After observation, there was no correlation between the two variables. Before and after treatment. There was no correlation between the two variables in the experimental group (P0.05). Conclusion low serum level of PIBF in early pregnancy is one of the factors leading to threatened abortion. The embryonic protective effect of Diproprogesterone may be achieved by upregulating the expression of serum PIBF. The serum PIBF level is helpful to the clinical diagnosis of threatened abortion and the evaluation of the curative effect of Diproprogesterone.
