【摘要】:目的:比較胚胎發(fā)育潛能低下的患者在凍融周期中移植不同時(shí)期胚胎的妊娠結(jié)局,以優(yōu)化其凍融策略。方法:回顧性分析2015年1月至10月于我院生殖醫(yī)學(xué)中心治療的57例常規(guī)體外受精(IVF)周期或卵胞漿內(nèi)單精子注射(ICSI)周期中胚胎發(fā)育潛能低下患者的凍融周期臨床資料。根據(jù)移植胚胎的時(shí)期分為分裂胚移植組(n=35)及囊胚移植組(n=22),比較兩組的臨床妊娠率、種植率及早期流產(chǎn)率。結(jié)果:凍融周期中,Day3分裂胚移植組與Day5囊胚移植組相比,臨床妊娠率(65.71%vs.63.63%)和種植率(50.00%vs.52.27%)相近;早期流產(chǎn)率雖無(wú)統(tǒng)計(jì)學(xué)差異,但分裂胚移植組的早期流產(chǎn)率有明顯高于囊胚移植組的趨勢(shì)(8.7%vs.0)。結(jié)論:凍融周期中,對(duì)胚胎發(fā)育潛能低下的患者建議優(yōu)選囊胚移植,以降低早期流產(chǎn)風(fēng)險(xiǎn)。本研究結(jié)果需擴(kuò)大樣本后進(jìn)一步探討,為優(yōu)化凍融周期的妊娠結(jié)局提供新思路。
[Abstract]:Objective: to compare the pregnancy outcomes of embryo transplants in different stages in freeze-thaw cycles in patients with low embryonic development potential. Methods: from January 2015 to October, 57 cases of IVF cycles or intracytoplasmic sperm injection (ICSI) were analyzed retrospectively. Clinical data of frozen-thawing cycle in patients with hypodevelopment of embryo development during ICSI cycle. According to the period of embryo transfer, they were divided into two groups: divided into divided embryo transfer group and blastocyst transplantation group (n ~ (35)) and blastocyst transplantation group (n ~ (22)). Results: the clinical pregnancy rate, implantation rate and early abortion rate of the two groups were compared with those of the Day5 blastocyst transplantation group. The clinical pregnancy rate (65.71 vs 63.63) and the planting rate (50.00v s.52.27) were similar. Although there was no statistical difference in the rate of early abortion, the rate of early abortion in the split embryo transplantation group was significantly higher than that in the blastocyst transplantation group. Conclusion: in the freeze-thaw cycle, the rate of early abortion is 8.7vs.0. In order to reduce the risk of early abortion, it is suggested to select blastocysts for patients with low embryonic development potential. The results of this study need to be further explored after expanding the sample to provide a new idea for optimizing the pregnancy outcome of freezing and thawing cycle.

【正文快照】: 胚胎凍融技術(shù)作為輔助生殖領(lǐng)域中的一項(xiàng)常規(guī)技術(shù),方法成熟,結(jié)果穩(wěn)定,是臨床治療中常規(guī)的保存生育力方法,既可減少IVF患者反復(fù)接受激素刺激的超排卵治療,又能夠有效的解決剩余胚胎的去向問(wèn)題,目前該技術(shù)已經(jīng)成為唯一被北美生殖醫(yī)學(xué)委員會(huì)認(rèn)可的臨床可開(kāi)展的生育力保存方法[1-5]
