胎兒長骨超聲測量與X線及裸骨的相關(guān)性研究

【摘要】:目的評價超聲測量正常胎兒長骨骨干長度及總長度與引產(chǎn)后X線測量長度及裸骨的相關(guān)性。方法選擇孕周20-38周423例長骨發(fā)育正常的胎兒,應(yīng)用二維超聲常規(guī)測量長骨骨干長度及其干骺端結(jié)構(gòu)的長度,并計(jì)算總長度。其中51例引產(chǎn)兒(20-32周)納入本次研究。引產(chǎn)后24小時內(nèi)將引產(chǎn)病例做尸體X線攝片檢查測量長骨長度,并測量其裸骨長度(包括干骺端結(jié)構(gòu))及裸骨X線長度,超聲測量總長度與引產(chǎn)后尸體X線攝片的長骨長度、裸骨長度及裸骨X線長度作對比。結(jié)合臨床資料將幾種測量結(jié)果做統(tǒng)計(jì)學(xué)分析。研究過程中,將部分引產(chǎn)后胎兒置于水中,模擬宮內(nèi)環(huán)境觀察胎兒長骨及干骺端結(jié)構(gòu)的形態(tài)學(xué)特征,部分新鮮裸骨同樣放置水中以高頻超聲觀察長骨回聲及其兩端干骺端結(jié)構(gòu)的特征。結(jié)果超聲觀察宮內(nèi)或離體胎兒肢體長骨骨干及干骺端結(jié)構(gòu)的形態(tài)學(xué)特征與X線及裸骨觀察結(jié)果基本一致,但長度及干骺端結(jié)構(gòu)可清晰顯示。超聲測量長骨骨干值及裸骨長度分別與尸體長骨X線測值比較,差異均具有統(tǒng)計(jì)學(xué)意義(P0.05),尸體長骨X線測值大于超聲測量長骨骨干值,小于裸骨長度:將超聲測量長骨總長度、裸骨X線測值分別與裸骨長度比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。相關(guān)性分析結(jié)果顯示超聲與裸骨測量長度存在高度正相關(guān)。結(jié)論超聲可精確測量骨干及干骺端結(jié)構(gòu)的長度。產(chǎn)前超聲、尸體X線攝片及裸骨三種方法測量結(jié)果不完全一致,但三者之間有一定相關(guān)性。尸體X線測量不能反映長骨真實(shí)的總長度,而超聲測量長度總長度與裸骨測量結(jié)果基本一致,且超聲測量與裸骨測量方法的相關(guān)性也較好,可更真實(shí)地反映胎兒長骨發(fā)育情況,從而有助于更準(zhǔn)確評價胎兒生長發(fā)育遲緩及短長骨。
[Abstract]:Objective to evaluate the ultrasound measurement of normal fetal long bone length and the total length of gestation with bare bone length and X-ray measurement after correlation. Methods 20-38 weeks of gestation in 423 cases of normal fetal long bone development, application of two-dimensional ultrasound measurement of long bone length and metaphyseal structure length, and calculate the total length. Among them 51 cases cited infants (20-32 weeks) were included in the study. Labor within 24 hours after the induction of case body X-ray measurement of long bone length, and measuring the bare bone length (including metaphyseal structure) and bare bone X-ray length, total length of ultrasonic measurement and autopsy long bone X-ray, bare bones the length and the length of bare bone X-ray compared with clinical data. Several measurement results were statistically analyzed. The course of the study, will be part of the induction of labor after fetal intrauterine environment simulation in water, observation of fetal long bone and metaphyseal structure form Features, some fresh bare bones also placed in water to observe the characteristics of the long bone metaphyseal structure echo and the two stem with high frequency ultrasound. Results the ultrasound observation of intrauterine fetal limb long bone or isolated diaphyseal and metaphyseal structure morphology and X-ray and bare bone observation results are basically the same, but the length and metaphyseal structure can be clearly displayed. Ultrasonic measurement of long bone value and bare bone length and body bone X-ray measurements were compared, the differences were statistically significant (P0.05), the body bone X-ray measured value is greater than the ultrasonic measurement of long bone values less than bare bone length: the total length of ultrasonic measurement of long bone, bare bone X-ray measurements were compared with the bare bone length, there were no significant differences (P0.05). The results of correlation analysis showed that there was a strong positive correlation with ultrasonic measuring the length of bare bone. Conclusion ultrasound can accurately measure the diaphyseal and metaphyseal structure. The length of prenatal ultrasound, The body X-ray and the bare bones of three measuring methods were not completely consistent, but there is a certain correlation between the three bodies. The total length of the long bone X-ray measurement can not reflect the true, and the length of the total length of ultrasonic measurement results are basically consistent with the bare bone, also a good correlation and ultrasonic measurement and measurement method of the bare bone, can reflect fetal long bone development, thus contributing to a more accurate assessment of fetal growth retardation and short long bones.
