【摘要】：目的探討胎盤植入相關(guān)超聲圖像特征在產(chǎn)前診斷中的價(jià)值。方法以70例在我院行產(chǎn)前超聲檢查疑似胎盤植入并在我院生產(chǎn)的孕婦為研究對(duì)象,回顧性分析患者病例、超聲報(bào)告及圖像。確診以生產(chǎn)中所見及病理結(jié)果為準(zhǔn)。結(jié)果在胎盤植入的產(chǎn)前診斷中,各圖像特征(胎盤后方間隙消失、胎盤內(nèi)廣泛腔隙、胎盤內(nèi)局灶腔隙、胎盤后方血流豐富、子宮膀胱壁界面毛糙)的靈敏度(特異度)分別為71.2%(18.2%)、35.6%(100%)、33.9%(36.4%)、42.4%(81.8%)、18.6%(90.9%)。在對(duì)產(chǎn)后子宮切除的預(yù)測(cè)中,各圖像特征的靈敏度(特異度)分別為63.2%(23.5%)、63.2%(82.4%)、21.1%(54.9%)、68.4%(72.5%)、36.8%(90.2%)。結(jié)論胎盤植入各圖像特征中,胎盤后方間隙消失靈敏度高而特異度低,容易造成誤診,而胎盤內(nèi)廣泛腔隙特異度高,且對(duì)預(yù)測(cè)產(chǎn)后子宮切除靈敏度高,在胎盤植入的產(chǎn)前診斷中具有重要價(jià)值。
[Abstract]:Objective to evaluate the value of placental accreta associated ultrasound imaging in prenatal diagnosis. Methods 70 pregnant women with suspected placenta accreta who were examined by prenatal ultrasound in our hospital were studied. The cases, ultrasonic reports and images were analyzed retrospectively. The diagnosis is based on the clinical and pathological findings. Results in the prenatal diagnosis of placenta accreta, the sensitivity (specificity) of all the imaging features (disappearance of posterior placenta space, extensive space in placenta, focal space in placenta, abundant blood flow behind placenta, rough interface of uterine bladder wall) were 71.2, 36.44.41.81.81.81.81.80.The sensitivity (specificity) was 90.9. In the prediction of postpartum hysterectomy, the sensitivity (specificity) of each image feature was 63.2 and 63.2, respectively. There were 54.9and 68.4and 54.9and 68.4and 90.2respectively. Conclusion among the features of placenta accreta, the disappearance of the posterior placenta space is sensitive and the specificity is low, it is easy to cause misdiagnosis, while the wide space in the placenta is highly specific, and it is highly sensitive to predict postpartum hysterectomy. It has important value in prenatal diagnosis of placenta accreta.
