化療對(duì)急性淋巴細(xì)胞白血病患兒骨代謝近期影響的初步探討

【摘要】:目的:探討化療對(duì)急性淋巴細(xì)胞白血。╝cute lymphoblastic leukemia, ALL)患兒骨代謝的近期影響,比較不同療程對(duì)骨代謝改變的差異。 方法:采取ALL患兒的116例血清樣本,利用電化學(xué)發(fā)光法檢測(cè)骨代謝指標(biāo):血清Ⅰ型膠原氨基端前肽(PINP)、Ⅰ型膠原交聯(lián)羧基端肽(CTX)。治療方案為2008-CCLG,時(shí)間點(diǎn)分別為:19例誘導(dǎo)緩解DOLP(DNR、VCR、L-Asp、Dex)療程前、后,14例CAM(CTX、Ara-C、6-MP)第一療程前、后,29例延遲強(qiáng)化DOLP(ADM、VCR、L-Asp、Dex)療程*后(標(biāo)危2例,中危13例,高危14例)、21例維持治療(標(biāo)、中危:6-MP+MTX/VD,高危:6-MP+MTX/CA/VD,)階段(半年)。同期健康體檢兒童18例設(shè)為對(duì)照組。(*注:中、高危組為延遲強(qiáng)化DOLPⅡ后) 結(jié)果:(1)誘導(dǎo)緩解DOLP療程前、后、對(duì)照組的PINP值分別為(182.47±112.83)ng/ml、(250.16±316.65)ng/ml、(549.42±285.50)ng/ml,療程前后PINP差別無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),但與對(duì)照組比較均有顯著性差異(P0.05); CTX值分別為(1.66±0.64)ng/ml、(1.95±1.08)ng/ml、(1.20±0.35)ng/ml,療程前后CTX比較差異有顯著性(P0.01),療程后CTX療程前CTX,且均大于對(duì)照組,差異有顯著性(P0.05)。(2)CAM療程前、后的PINP值分別為(617.49±430.83)ng/ml、(665.86±240.02)ng/ml,兩組比較無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。療程前、后PINP分別與對(duì)照組比較,均無(wú)顯著性差異(P0.05);療程前、后CTX分別為(1.90±0.72)ng/ml、(1.79±0.38)ng/ml,兩組比較無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。分別與對(duì)照組比較,,差別均有顯著性(P0.05)。(3)延遲強(qiáng)化DOLP療程后標(biāo)+中危組與高危組PINP分別為(357.57±219.37)ng/ml、(319.30±253.44)ng/ml,兩組比較無(wú)顯著差異(P0.05),但兩組分別與對(duì)照組比較,均有顯著性差異(P0.05);標(biāo)+中危組與高危組CTX分別為(1.51±0.62)ng/ml、(1.63±0.31)ng/ml,兩組比較無(wú)顯著差異(P0.05),但兩組分別與對(duì)照組比較,均有統(tǒng)計(jì)學(xué)意義(P0.05)。(4)維持期PINP、CTX分別為(631.89±215.54)ng/ml、(1.23±0.31)ng/ml,與對(duì)照組比較均無(wú)顯著差異(P0.05)。 結(jié)論:(1)ALL患兒化療前存在骨代謝異常,表現(xiàn)為骨合成抑制,骨破壞增強(qiáng),考慮與白血病細(xì)胞浸潤(rùn)相關(guān);(2)ALL患兒化療期間存在骨代謝異常,與正常對(duì)照相比,化療早期以骨破壞增強(qiáng)為主,骨合成抑制;至維持治療期,骨合成、骨分解代謝基本平衡;(3)不同療程對(duì)骨代謝影響不同。誘導(dǎo)緩解DOLP療程促進(jìn)骨破壞,CAM療程對(duì)骨代謝無(wú)明顯影響,化療后期不同危險(xiǎn)組患兒的骨代謝無(wú)顯著差別。
