【摘要】：目的:隨訪總結(jié)軟石膏應(yīng)用于閉合復(fù)位髖人位石膏固定法治療六月齡前髖關(guān)節(jié)發(fā)育不良患兒的療效及預(yù)后情況,評估其應(yīng)用價值。方法:收集2011~2015年在我院治療的共22例六月齡前DDH住院病例,均采用閉合復(fù)位髖人位石膏固定治療。長期隨訪其預(yù)后情況并應(yīng)用Mckay功能評定標(biāo)準(zhǔn)和Severin評分標(biāo)準(zhǔn)對患兒的髖關(guān)節(jié)功能和影像學(xué)資料進(jìn)行評估。同時對患兒末次隨訪時的年齡、髖臼指數(shù)、有無并發(fā)癥的發(fā)生等進(jìn)行分析。結(jié)果:經(jīng)10個月至32個月不等的隨訪,平均隨訪時間為18月,22例25髖最終均達(dá)到Mckay功能評定標(biāo)準(zhǔn)優(yōu)級標(biāo)準(zhǔn);21例24髖達(dá)到Severin評分標(biāo)準(zhǔn)優(yōu)級標(biāo)準(zhǔn),1例1髖達(dá)到Severin評分標(biāo)準(zhǔn)良級標(biāo)準(zhǔn),優(yōu)良率100%,其中優(yōu)95.5%,良4.5%;且22例患兒經(jīng)治療后末次隨訪時髖臼指數(shù)均將至正常水平,無一例后期須行骨性手術(shù),且石膏固定期間無一例發(fā)生石膏壓瘡、股骨頭缺血性壞死(AVN)。結(jié)論:閉合復(fù)位髖人位軟石膏固定是治療六月齡前髖關(guān)節(jié)發(fā)育不良患兒的一種切實有效的方法,其臨床隨訪療效及預(yù)后令人滿意,且能有效減少石膏壓瘡、股骨頭缺血性壞死等并發(fā)癥的發(fā)生,值得推廣使用。
[Abstract]:Aim: to evaluate the efficacy and prognosis of anhydrite in the treatment of 6-month-old children with hip dysplasia. Methods: 22 cases of DDH before 6 months of age were treated in our hospital from 2011 to 2015. All patients were treated with closed reduction and hip plaster fixation. The prognosis was followed up for a long time and the hip joint function and imaging data were evaluated by Mckay and Severin scoring criteria. At the same time, the age, acetabular index and complications of the last follow-up were analyzed. Results: after 10 months to 32 months follow-up, the average follow-up time was 18 months, 22 cases (25 hips) finally reached the Mckay functional evaluation standard. Twenty-one cases (24 hips) reached the Severin score standard, and one case (1 hip) reached the Severin score standard. The excellent and good rate was 100%, among which the excellent rate was 95.5% and the good was 4.5%. The acetabular index would reach normal level at the last follow-up of 22 cases after treatment, none of them needed bony operation at the later stage, and none of them had plaster pressure sore and avascular necrosis of the femoral head (AVN).) during the plaster fixation period. Conclusion: hip anhydrite fixation with closed reduction is a practical and effective method for the treatment of 6-month-old children with hip dysplasia. The clinical follow-up results and prognosis are satisfactory, and it can effectively reduce plaster pressure sores. The occurrence of complications such as avascular necrosis of the femoral head is worth popularizing and using.
