?诘貐^(qū)小兒呼吸道病原體檢測(cè)結(jié)果分析

【摘要】:目的:對(duì)海南省中醫(yī)院、?谑腥嗣襻t(yī)院住院及門(mén)診的小兒呼吸道感染患者檢測(cè)9種病原體抗體并進(jìn)行結(jié)果分析,探討?诘貐^(qū)小兒呼吸道患者9種病原體的流行分布情況,不同年齡組、不同性別、不同季節(jié)病原體的檢出情況,為海口地區(qū)小兒呼吸道疾病預(yù)防策略制定、臨床診斷及治療提供病原學(xué)實(shí)驗(yàn)室依據(jù)。 方法:收集2013年1月至2013年12月在海南省中醫(yī)院、?谑腥嗣襻t(yī)院住院及門(mén)診小兒呼吸道感染患者1144例,按照小兒的年齡分期標(biāo)準(zhǔn),分為4個(gè)年齡組。應(yīng)用間接免疫熒光法同時(shí)對(duì)9種病原體行血清IgM抗體聯(lián)合檢測(cè)及分析研究,這9種病原體分別為嗜肺軍團(tuán)菌(LP),肺炎支原體(MP),Q熱立克次體(COX),肺炎衣原體(CPm),腺病毒(AdV),呼吸道合胞病毒(RSV),甲型流感病毒(INFA)和乙型流感病毒(INFB),副流感病毒(PIVs)。 結(jié)果:1.1144例患者血清樣本中,檢出IgM抗體陽(yáng)性536例,陽(yáng)性率為46.85%。單項(xiàng)感染339例,陽(yáng)性率為29.63%。 2.單項(xiàng)感染排前三位的分別是MP,陽(yáng)性率為22.5%(258/1144)、INFB,陽(yáng)性率為4.11%(47/1144)、LP陽(yáng)性率為2.27%(26/1144);旌细腥197例,以MP+INFB感染為主,占10.23%(117/1144):其次為L(zhǎng)P+MP混合感染,陽(yáng)性率為2.62%(30/1144);第三位是LP+MP+INFB混合感染,陽(yáng)性率為1.92%(22/1144);其陽(yáng)性率和構(gòu)成比均不同(P0.05)。 3.4個(gè)年齡組陽(yáng)性率隨年齡增加而增加,學(xué)齡前期組感染陽(yáng)性率高,為73.06%(198/270),其次為幼兒組,陽(yáng)性率為63.00%(126/200),第三位為嬰兒后期組,陽(yáng)性率為48.93%(183/374),各組陽(yáng)性率及構(gòu)成比均不相同(P0.05)。 4.1144個(gè)病例中,檢出536例病原體抗體呈陽(yáng)性反應(yīng),其中男性326例,陽(yáng)性率為41.85%,構(gòu)成比為0.61;女性210例,陽(yáng)性率為57.53%,男性和女性感染的陽(yáng)性率和構(gòu)成比均不同(P0.05)。在男性混合感染中,陽(yáng)性率排在前三位的分別為MP+INFB(陽(yáng)性率為7.96%)、LP+MP(陽(yáng)性率為2.70%)和LP+MP+INFB(1.41%)。在女性混合感染中,陽(yáng)性率排在前三位的分別為MP+INFB(陽(yáng)性率為15.07%)、LP+MP+INFB(陽(yáng)性率為3.01%)和LP+MP(陽(yáng)性率為2.47%),三種組合的病原體間感染陽(yáng)性率及構(gòu)成比均各不同(P0.05),同種病原體在不同性別中感染陽(yáng)性率各不同(P0.05)。 5.四個(gè)季節(jié)中,夏季感染陽(yáng)性率高達(dá)51.84%,構(gòu)成比為0.32;秋季為50.16%,春季為48.34,冬季為31.16%。經(jīng)卡方檢驗(yàn),與冬季相比,其他三個(gè)季節(jié)陽(yáng)性率和構(gòu)成比均顯著上升(P0.05),而夏季與秋季間陽(yáng)性率與構(gòu)成比均無(wú)顯著差異(P0.05)。在春季,9種病原體感染陽(yáng)性率前三位分別為MP、MP+INFB、MP+LP。在夏季,9種病原體感染陽(yáng)性率前三位分別為MP、MP+INFB、INFB。在秋季,9種病原體感染陽(yáng)性率前三位分別為MP、MP+INFB、INFB。在冬季,9種病原體感染陽(yáng)性率前三位分別為MP、LP、INFB。選擇感染陽(yáng)性率較高的4種感染類(lèi)型,分別為MP、LP、INFB、MP+INFB,分析其在各季節(jié)中陽(yáng)性率差異。MP感染的陽(yáng)性率在夏季顯著上升(P0.05),LP感染的陽(yáng)性率在冬季顯著上升(P0.05),INFB感染的陽(yáng)性率在秋季顯著上升(P0.05),MP+INFB感染的陽(yáng)性率在秋季顯著上升(P0.05)。 結(jié)論:1.?诘貐^(qū)9種小兒呼吸道病原體,單項(xiàng)感染以肺炎支原體感染為常見(jiàn),混合感染以肺炎支原體合并乙型流感病毒感染為主,單項(xiàng)感染多于混合感染。 2.9種小兒呼吸道病原體感染在不同年齡段的陽(yáng)性率有顯著不同,學(xué)齡前期組最高,嬰兒前期組最低。 3.男性和女性感染的陽(yáng)性率和構(gòu)成比均不同。 4.9種小兒呼吸道病原體流行還與季節(jié)有關(guān)。 5.該研究為?诘貐^(qū)預(yù)防策略制定、臨床診斷、合理用藥提供病原學(xué)實(shí)驗(yàn)室依據(jù)。
[Abstract]:Objective: To study the prevalence and distribution of 9 pathogens in children with respiratory tract infection in the hospital of Hainan, the hospital of the people's hospital in Haikou, and in the clinic, and to study the distribution of 9 pathogens in the children's respiratory tract of Haikou. The detection of pathogens in different seasons provides a basis for the development, clinical diagnosis and treatment of children's respiratory diseases in Haikou.
