151例糖尿病母親新生兒早期并發(fā)癥臨床分析

【摘要】:目的:分析妊娠合并糖尿病及糖尿病母親孕期血糖控制情況對(duì)新生兒的影響,為臨床管理糖尿病母親新生兒早期并發(fā)癥提供參考依據(jù)。 方法:選取2009年1月~2011年12月就診于吉林大學(xué)白求恩第一醫(yī)院的11191例患兒中確診為糖尿病母親新生兒(infant of diabetic mothers,IDMS)共151例作為觀察對(duì)象,采取回顧性分析方法,觀察IDMS發(fā)病率情況及早期并發(fā)癥發(fā)生的情況。將151例IDMS分為4組:1、根據(jù)胎齡(gestational age,GA)不同劃分為早產(chǎn)組(31w≤GA≤36+6w)73例,足月組(37w≤GA≤40+4w)78例。比較IDMS兩組并發(fā)癥的異同點(diǎn)。2、根據(jù)產(chǎn)科提供的151例IMDS母親的孕期血糖控制情況,分為血糖控制良好組(觀察1組)72例和血糖控制不良組(觀察2組)79例。然后再比較母親糖尿病妊娠期血糖控制情況與IDMS早期并發(fā)癥的關(guān)系。 結(jié)果顯示:1、三年來我院IDMS發(fā)病率依次為2009年0.93%,2010年1.15%,,2011年1.90%,其中IDMS早產(chǎn)兒發(fā)病率依次為2009年0.93%,2010年1.34%,2011年3.05%,IDMS足月兒發(fā)病率依次為2009年0.93%,2010年1.15%,2011年1.90%。2、IDMS早期并發(fā)癥中,大于胎齡兒84例(55.6%),其中巨大兒51例(33.8%),高膽紅素血癥57例(37.7%),低血糖癥46例(30.5%),新生兒濕肺41例(27.2%),RDS19例(12.6%),產(chǎn)傷17例(11.3%),宮內(nèi)窘迫16例(10.6%),心肌肥厚13例(8.6%),低鈣血癥13例(8.6%),窒息10例(6.6%),小于胎齡兒10例(6.6%),先天性畸形10例(6.6%),顱內(nèi)出血5例(3.3%),紅細(xì)胞增多癥2例(1.3%),低血糖腦病2例(1.3%),血栓1例(0.7%)。3、IDMS足月組大于胎齡兒發(fā)病率高于早產(chǎn)組,P0.05,差異具有統(tǒng)計(jì)學(xué)意義。IDMS早產(chǎn)組小于胎齡兒、RDS發(fā)病率與足月組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。4、糖尿病母親妊娠期血糖控制不良組(觀察2組)大于胎齡兒、低血糖癥、心肌肥厚、產(chǎn)傷、窒息發(fā)病率明顯高于血糖控制良好組(觀察1組),P0.05,差異有統(tǒng)計(jì)學(xué)意義;而小于胎齡兒、高膽紅素血癥、RDS、宮內(nèi)窘迫、先天性畸形、低鈣血癥、新生兒濕肺發(fā)病率與血糖控制良好組(觀察1組)比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論:1、三年來,我院收治的IDMS發(fā)病率逐年上升,其中以IDMS早產(chǎn)兒發(fā)病率逐漸增多占優(yōu)勢(shì),提示早產(chǎn)可能為糖尿病母親妊娠的不良后果之一。2、151例IDMS早期并發(fā)癥中,大于胎齡兒發(fā)生率最高,其它并發(fā)癥發(fā)生率由高到低依次為:高膽紅素血癥、低血糖癥、新生兒濕肺、RDS、產(chǎn)傷、宮內(nèi)窘迫、心肌肥厚、低鈣血癥、窒息、小于胎齡兒、先天性畸形、顱內(nèi)出血、紅細(xì)胞增多癥、低血糖腦病、血栓。3、IDMS足月組大于胎齡兒發(fā)生率明顯高于早產(chǎn)組;母親糖尿病對(duì)早產(chǎn)兒小于胎齡兒及足月兒RDS發(fā)病率有一定影響。4、母親妊娠期血糖控制良好可減少IDMS早期并發(fā)癥發(fā)生。
