【摘要】：目的探討先天性直腸肛門畸形術(shù)后大便功能障礙原因,分析其相關(guān)危險(xiǎn)因素。方法回顧性研究1994年1月至2014年1月我院手術(shù)治療先天性直腸肛門畸形306例患兒資料,根據(jù)其術(shù)后是否存在大便功能障礙分為功能障礙組(87例)和功能良好組(219例)。從年齡、性別、臨床分型、手術(shù)方式、手術(shù)次數(shù)、合并畸形、手術(shù)時(shí)間、合并瘺管等相關(guān)因素進(jìn)行Logistic回歸分析,探討導(dǎo)致術(shù)后大便障礙的危險(xiǎn)因素。結(jié)果306例患兒資料,其中有87例患兒術(shù)后出現(xiàn)不同程度的大便功能障礙,約占28.33%。95例患兒伴發(fā)畸形,占44.17%,其中泌尿系統(tǒng)畸形有34人次,占35.79%；生殖系統(tǒng)畸形有31人次,占32.63%；心血管畸形有12人次,占12.63%；神經(jīng)系統(tǒng)畸形有8人次,占8.42%；運(yùn)動(dòng)系統(tǒng)畸形6人次,占6.32%；其他畸形4人次,占4.21%。單因素、多因素Logistic回歸分析均顯示臨床分型、手術(shù)方式、手術(shù)次數(shù)、合并畸形4個(gè)因素與術(shù)后大便功能障礙有顯著相關(guān)性(P0.05),其余因素均無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論先天性直腸肛門畸形術(shù)后大便功能障礙的發(fā)生與臨床分型、手術(shù)方式、手術(shù)次數(shù)、合并畸形有著明顯相關(guān)性。
[Abstract]:Objective to investigate the causes of defecation dysfunction after operation of congenital anorectal malformation and analyze its related risk factors. Methods from January 1994 to January 2014, 306 cases of congenital rectal and anal malformation were treated by operation in our hospital. According to whether there were defecation dysfunction after operation, we divided them into two groups: 87 cases of dysfunction group and 219 cases of good function group. Logistic regression analysis was carried out on age, sex, clinical type, operation mode, times of operation, complicated malformation, operation time and fistula. The risk factors of postoperative defecation disorder were discussed. Results among 306 cases, 87 cases had different degree of defecation dysfunction, accounting for 28.33% of 95 cases (44.17%), among which 34 cases (35.79%) had urinary system malformation. There were 31 malformations of the reproductive system (32.63), 12 of the cardiovascular malformations (12.63), 8 of the malformations of the nervous system (8.42), 6 of the deformities of the motor system (6.32); Other deformities were 4 times (4.21%). Univariate and multivariate Logistic regression analysis showed that there were significant correlations between the four factors of clinical classification, operation mode, times of operation and complicated deformity (P0.05), but no statistical significance in the other factors. Conclusion the occurrence of postoperative defecation dysfunction in patients with congenital anorectal malformation has a significant correlation with clinical type, operation mode, times of operation and complicated malformation.
