64-MSCT在小兒復雜型先天性心臟病診斷中的應用

【摘要】:目的 探討64排螺旋CT(64-muti detector spiral computed tomography,64-MSCT)在小兒復雜型先天性心臟病診斷中的價值。 方法 回顧性分析56例小兒復雜型先天性心臟病患者的完整資料。所有患兒均采用彩色多普勒超聲和64-MSCT檢查,并經(jīng)常規(guī)心血管造影檢查和/或手術(shù)證實。以手術(shù)、心血管造影結(jié)果為標準,對比彩色多普勒超聲與64-MSCT對小兒復雜型先天性心臟病的診斷價值。 結(jié)果56例復雜型先天性心臟病共164個畸形,其中64-MSCT診斷正確151處,診斷符合率為92.07%,彩色多普勒超聲診斷正確130處,診斷符合率為79.27%。其中心內(nèi)結(jié)構(gòu)畸形實際存在59處,64-MSCT診斷正確49處,誤診8處,漏診2處,診斷符合率為83.05%;彩色多普勒超聲診斷正確57處,誤診2處,診斷符合率為96.61%。64-MSCT與彩色多普勒超聲對診斷小兒復雜型先天性心臟病的心內(nèi)結(jié)構(gòu)畸形差異有統(tǒng)計學意義。56例患者實際存在心外畸形結(jié)構(gòu)105處,64-MSCT診斷正確102處,誤診3處,診斷符合率為97.14%;彩色多普勒超聲診斷正確73處,漏診32處,診斷符合率為69.52%,兩種檢查方法對診斷心外結(jié)構(gòu)畸形差異有統(tǒng)計學意義。 結(jié)論 64-MSCT對小兒復雜型先心病心外畸形結(jié)構(gòu)的診斷符合率高于彩色多普勒超聲,但是對心內(nèi)畸形結(jié)構(gòu)的診斷符合率略低于彩色多普勒超聲。64-MSCT可以作為小兒復雜型先心病檢查方法之一
[Abstract]:Objective to investigate the value of 64-slice spiral CT (64-muti detector spiral computed tomography,64-MSCT) in the diagnosis of complex congenital heart disease in children. Methods the complete data of 56 children with complicated congenital heart disease were retrospectively analyzed. All children were examined by color Doppler ultrasound and 64-MSCT, and confirmed by routine angiocardiography and / or surgery. The diagnostic value of color Doppler ultrasonography and 64-MSCT in children with complicated congenital heart disease was compared. Results there were 164 deformities in 56 cases of complex congenital heart disease, of which 64-MSCT was correctly diagnosed in 151 cases, the diagnostic coincidence rate was 92.07 and the color Doppler ultrasound was correct in 130 cases, and the diagnostic coincidence rate was 79.27. There were 59 structural deformities in the center, 49 were correctly diagnosed by 64-MSCT, 8 were misdiagnosed and 2 missed. The diagnostic coincidence rate was 83.05; Color Doppler ultrasound diagnosed 57 cases correctly and misdiagnosed 2 cases. The diagnostic coincidence rate was that 96.61%.64-MSCT and color Doppler ultrasound were significantly different in the diagnosis of complex congenital heart disease in children. There were 105 extracardiac malformations in 56 patients. 64-MSCT diagnosis was correct in 102 cases and misdiagnosed in 3 cases. The diagnostic coincidence rate was 97.14%. There were 73 correct diagnosis and 32 missed diagnosis by color Doppler ultrasound, and the diagnostic coincidence rate was 69.522.There was significant difference between the two methods in the diagnosis of extracardiac structural deformity. Conclusion the diagnostic accuracy of 64-MSCT in the diagnosis of extracardiac malformation in children with complex congenital heart disease is higher than that of color Doppler ultrasound. However, the diagnostic coincidence rate of intracardiac malformations was slightly lower than that of color Doppler sonography. 64-MSCT can be used as one of the methods for the diagnosis of complex congenital heart disease in children.
