【摘要】：目的評估簡化曲普瑞林(達必佳，促性腺激素釋放激素類似物)激發(fā)試驗在女童下丘腦-垂體-性腺軸(HPGA)啟動中的臨床意義。 方法在108例(性早熟87例、矮小癥13例和青春期預(yù)測終身高矮小8例)女童中，HPGA啟動者79例，未啟動者29例。所有患兒均行曲普瑞林激發(fā)試驗(08:30a.m.，0.1mg/次，皮下注射)，于0、20、40和60min采血樣各2mL者為90例；在另18例中，16例采血時間為0、30和60min，，1例為0和40min，1例為0和60min。應(yīng)用免疫化學(xué)發(fā)光分析法(ICMA)檢測促黃體生成素(LH)和卵泡刺激素(FSH)。 結(jié)果在90/108例女童中，啟動者與未啟動者的LH峰值(PLH)分別為(20.24±15.39)和(2.56±1.03) IU/L， PLH/FSH峰值(PFSH)分別為(1.35±0.94)和(0.32±0.42)，P均≤0.01；曲普瑞林激發(fā)試驗40min LH(91/108例)、60min LH(107/108例)、PLH(90/108例)診斷HPGA啟動的受試者工作特性曲線下面積分別為0.990、0.982和0.990；當Youden指數(shù)最大時，即40min LH、60min LH、PLH分別≥4.09、4.04和4.22IU/L時，其診斷HPGA啟動的靈敏度分別為93.9%、96.2%和96.9%，對應(yīng)的特異度均為100.0%；當40和60min的LH≥4.22IU/L時，其診斷HPGA啟動的靈敏度分別為90.9%和96.2%，對應(yīng)的特異度均為100.0%。當PLH/PFSH≥0.47時，其診斷HPGA啟動的靈敏度和特異度分別為83.1%和96.0%。 結(jié)論1h的曲普瑞林激發(fā)試驗(ICMA檢測法)為評估女童HPGA啟動的可靠實驗室指標，其40或60min單次血促性腺激素診斷價值與促性腺激素峰值接近。
[Abstract]:Objective to evaluate the clinical significance of simplified triplatin stimulation test in the initiation of hypothalamus-pituitary-gonadal axis (HPGA) in girls. Methods among 108 girls (87 cases of precocious puberty, 13 cases of dwarfism and 8 cases of predicted lifetime high dwarf in puberty), 79 cases were HPGA promoter and 29 cases were not.
