不同胎齡及出生體質(zhì)量早產(chǎn)兒早期神經(jīng)發(fā)育的縱向研究

【摘要】:目的探討早產(chǎn)兒早期神經(jīng)發(fā)育趨勢(shì)。方法隨訪早產(chǎn)兒及足月兒于12、24月齡時(shí)(早產(chǎn)兒為校正年齡),以Bayley嬰幼兒發(fā)育量表評(píng)估的智力發(fā)育指數(shù)(MDI)及運(yùn)動(dòng)發(fā)育指數(shù)(PDI)的差異。結(jié)果 12月齡時(shí),出生胎齡32周、32~33~(+6)周、34~36~(+6)周早產(chǎn)兒各組與足月兒組之間按校正年齡(CA)的PDI得分差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.820);足月兒和出生胎齡34~36~(+6)周組按實(shí)際年齡(UCA)的MDI和PDI得分均高于32周和32~33~(+6)周組,足月兒組的PDI得分高于34~36~(+6)周組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。不同出生體質(zhì)量早產(chǎn)兒與足月兒組之間按CA的PDI得分比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.166),≥2 500 g早產(chǎn)兒及足月兒組按UCA的MDI和PDI得分高于出生體質(zhì)量1 500 g、1 500~1 999 g及2 000~2 499 g早產(chǎn)兒組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。24月齡時(shí),按UCA的MDI得分足月兒組高于出生胎齡32周、32~33~(+6)周、34~36~(+6)周早產(chǎn)兒組,也高于出生體質(zhì)量1 500 g、1 500~1 999 g及2 000~2 499 g早產(chǎn)兒組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。1 500g早產(chǎn)兒組按UCA的MDI曲線呈下降趨勢(shì),而按UCA的PDI曲線呈明顯上升趨勢(shì)。結(jié)論早產(chǎn)兒CA12月齡、24月齡時(shí)神經(jīng)發(fā)育基本達(dá)同齡足月兒水平。
