【摘要】：目的觀察奧曲肽注射液聯(lián)合水解奶粉對壞死性小腸結(jié)腸炎新生兒營養(yǎng)狀態(tài)、免疫功能的影響。方法 90例壞死性小腸結(jié)腸炎新生患兒分為試驗(yàn)組和對照組,各45例。所有患兒均用30 g·kg~(-1)的奧曲肽注射液進(jìn)行治療,對照組喂養(yǎng)常規(guī)配方奶粉,試驗(yàn)組喂養(yǎng)水解配方奶粉,根據(jù)患兒的實(shí)際情況進(jìn)行哺乳,連續(xù)治療1周。治療后,比較2組患兒的營養(yǎng)狀態(tài)(血清白蛋白、血清前蛋白)、免疫功能(CD_4~+細(xì)胞、CD_8~+細(xì)胞、免疫球蛋白G、免疫球蛋白A、免疫球蛋白M)及胃腸道激素的變化,并比較2組患兒的臨床療效。結(jié)果治療后,試驗(yàn)組患兒的總有效率為93.33%(42/45例),對照組為77.78%(35/45例,P0.05)。治療后,試驗(yàn)組患兒的血清白蛋白為(41.51±5.59)g·L~(-1),血清前蛋白含量為(112.37±15.35)g·L~(-1);對照組血清白蛋白為(32.59±4.42)g·L~(-1),血清前蛋白含量為(103.24±13.67)g·L~(-1)(P0.05)。治療后,試驗(yàn)組胃泌素為(292.51±35.87)pg·mL~(-1),胃動素為(121.81±16.38)pg·mL~(-1);對照組胃泌素為(231.41±29.15)pg·mL~(-1),胃動素為(90.47±11.36)pg·mL~(-1)(P0.05)。治療后,試驗(yàn)組CD_4~+、CD_8~+為(28.30±2.62)%,(21.90±2.45)%;對照組為(32.03±2.13)%,(27.80±2.54)%(P0.05)。試驗(yàn)組免疫球蛋白G、免疫球蛋白A、免疫球蛋白M分別為(13.16±1.03),(2.94±0.23),(1.46±0.15)mg·dL~(-1),對照組分別為(11.13±0.41),(1.97±0.16),(1.22±0.25)mg·dL~(-1)(P0.05)。2組患兒治療過程中均沒有發(fā)生藥物不良反應(yīng)。結(jié)論奧曲肽聯(lián)合水解奶粉喂養(yǎng)新生兒壞死性小腸結(jié)腸炎,能明顯的改善患兒的營養(yǎng)狀態(tài)和免疫功能,提高患兒的腸胃激素含量,提高治療效果。
[Abstract]:Objective To observe the effect of octreotide injection combined with hydrolyzed milk powder on nutritional status and immune function of neonates with necrotizing enterocolitis.Methods 90 neonates with necrotizing enterocolitis were divided into experimental group and control group, 45 cases in each group. After treatment, the nutritional status (serum albumin, serum prealbumin), immune function (CD_4~+ cells, CD_8~+ cells, immunoglobulin G, immunoglobulin A, immunoglobulin M) and gastrointestinal hormones were compared between the two groups. Results After treatment, the total effective rate was 93.33% (42/45 cases) in the experimental group and 77.78% (35/45 cases, P 0.05) in the control group. After treatment, the serum albumin of the experimental group was (41.51 (5.59) g (-1), the serum prealbumin content was (112.37 (15.35) g (-1), the serum albumin of the control group was (32.59 (4.42) g (-1), and the serum albumin of the control group After treatment, the gastrin was (292.51 [(35.87) PG mg mL ~(-1) and motilin was (121.81 [(16.81 [16.38) PG mL ~ (-1); the gastrin was (231.41 [29.15) PG mL ~ (-1) in control group (231.41 [(29.15) PG ~ (-1), motilin was (90.47 [11.36) PG mL ~ (-1)(90.47 [(90.47 [11.36) PG [11.36) PG mL ~ (-1)(P 0.05). After treatment, the levels of motilin were (121, 121.81(28.30 (+ 2.62)%, (21.90)%. Immunoglobulin G, immunoglobulin A, and immunoglobulin M were (13.16 (+ 1.03), (2.94 (+ 0.23), (1.46 (+ 0.15) mg (- 1) respectively in the experimental group and (11.13 (+ 0.41), (1.97 (+ 0.16), (1.22 (+ 0.25) mg (- 1) (P 0.05)) in the control group. Conclusion Octreotide combined with hydrolyzed milk powder can obviously improve the nutritional status and immune function of neonates with necrotizing enterocolitis, increase the content of gastrointestinal hormones and improve the therapeutic effect.
