【摘要】：目的:觀察宣肺調(diào)脾飲治療兒童上氣道咳嗽綜合征(肺脾不調(diào)型)的臨床療效,客觀評價其有效性。方法:選擇符合西醫(yī)診斷標(biāo)準(zhǔn)和中醫(yī)證候標(biāo)準(zhǔn)的上氣道咳嗽綜合征(肺脾不調(diào)型)患兒60例,隨機(jī)分為治療組30例,對照組30例。對照組給予布地奈德鼻噴霧劑治療,治療組給予宣肺調(diào)脾飲治療,兩組均治療2周評價療效。治療開始前與結(jié)束后,分別記錄咳嗽癥狀積分、中醫(yī)證候積分和萊斯特咳嗽問卷積分,通過對比兩組咳嗽消失率、主癥積分、中醫(yī)證候療效及萊斯特咳嗽問卷積分,客觀評價宣肺調(diào)脾飲的臨床療效。結(jié)果:1.咳嗽消失率比較:治療組30例患兒中,臨床控制10例,顯效7例,有效11例,無效2例,總有效率93.3%;對照組30例患兒中,臨床控制6例,顯效12例,有效5例,無效7例,總有效率76.7%。兩組總有效率比較,經(jīng)統(tǒng)計學(xué)分析,差異無統(tǒng)計學(xué)意義(P0.05),提示兩組在咳嗽消失率方面療效相當(dāng)。2.主癥積分及中醫(yī)證候療效比較:(1)兩組組內(nèi)治療前后主癥比較,積分明顯降低,差異有統(tǒng)計學(xué)意義(P0.05);(2)兩組治療后組見主癥積分比較,除“咳嗽”一項積分差異無統(tǒng)計學(xué)意義(P0.05),其余主癥積分差異均有統(tǒng)計學(xué)意義(P0.05)。(3)兩組治療后中醫(yī)證候療效比較:治療組總有效率96.7%,對照組總有效率73.3%,差異有顯著統(tǒng)計學(xué)意義(P0.01)。3.萊斯特咳嗽問卷積分:(1)兩組治療后萊斯特咳嗽問卷積分較治療前均有明顯改善,統(tǒng)計學(xué)有顯著性差異(P0.01);(2)治療組治療后萊斯特咳嗽問卷積分改善值與對照組比較,差異有顯著統(tǒng)計學(xué)意義(P0.01)。結(jié)論:宣肺調(diào)脾飲治療肺脾不調(diào)型兒童上氣道咳嗽綜合征,療效確切,能明顯改善患兒局部癥狀和體征,其對患兒咳嗽的改善與布地奈德鼻噴霧劑相當(dāng),中醫(yī)證候療效優(yōu)于布地奈德鼻噴霧劑。
[Abstract]:Objective: to observe the clinical effect of Xuanfei Tiaopi decoction (XFTiao Yin) in treating children's upper airway cough syndrome. Methods: sixty children with upper airway cough syndrome (lung and spleen disorder) were randomly divided into treatment group (n = 30) and control group (n = 30). The control group was treated with budesonide nasal spray, the treatment group was treated with Xuanfei Tiaopi decoction, and the two groups were treated with budesonide nasal spray for 2 weeks. Before and after the treatment, cough symptom score, TCM syndrome score and Leicester cough questionnaire score were recorded, respectively. The cough disappearance rate, the main symptom score, the curative effect of TCM syndrome and the Lester cough questionnaire score were compared between the two groups. Objective to evaluate the clinical effect of Xuanfei Tiaopi decoction. The result is 1: 1. There were 10 cases of clinical control, 7 cases of marked effect, 11 cases of effective, 2 cases of ineffective, and 93.3% of total effective rate in the treatment group, while in the control group, there were 6 cases of clinical control, 12 cases of remarkable effect, 5 cases of effective and 7 cases of failure. The total effective rate is 76. 7%. The total effective rate of the two groups was compared by statistical analysis, the difference was not statistically significant (P0.05), suggesting that the two groups in cough disappearance rate is equivalent. 2. 2. Main symptom score and TCM syndrome efficacy comparison: (1) the main symptoms of the two groups before and after treatment, the score significantly decreased, the difference was statistically significant (P0.05); (2) after treatment in the two groups to see the main symptom score comparison, Except "cough" one integral difference has no statistical significance (P0.05), the other main symptom integral difference all has the statistical significance (P0.05). (3) two groups after the treatment traditional Chinese medicine syndrome curative effect comparison: the treatment group total effective rate 96.7%, the control group total effective rate 73.3%, the difference has the statistical significance (P0.01) .3. The score of Leicester cough questionnaire: (1) after treatment, the score of Leicester cough questionnaire in the two groups was significantly improved compared with that before treatment (P0.01); (2). The improvement value of Leicester cough questionnaire score in the treatment group was higher than that in the control group after treatment (P0.01); (2). The difference was statistically significant (P0.01). Conclusion: Xuanfei Tiaopi decoction is effective in treating upper airway cough syndrome of children with lung and spleen disorder, and can obviously improve the local symptoms and signs of children. The improvement of cough in children is similar to that of budesonide nasal spray. The curative effect of TCM syndrome is better than budesonide nasal spray.
