【摘要】：目的探討兒童孟氏骨折的手術(shù)治療方法及臨床療效。 方法對我院2003年3月至2011年3月20例兒童孟氏骨折病例進(jìn)行隨訪分析,手法復(fù)位失敗的新鮮兒童孟氏骨折采用尺骨復(fù)位內(nèi)固定環(huán)狀韌帶修補(bǔ)術(shù)治療。陳舊性兒童孟氏骨折患者,采用尺骨截骨內(nèi)固定,尺骨切開復(fù)位環(huán)狀韌帶重建術(shù)治療。術(shù)后定期復(fù)查X線片,觀察骨折愈合情況,并檢查前臂功能恢復(fù)情況。采用Mackay臨床功能評定表評估前臂功能恢復(fù)情況。 結(jié)果術(shù)后18例獲得隨訪,失訪2例,隨訪病例所有骨折均愈合。按Mackay評分：優(yōu)：8例,良：9例,差：1例;優(yōu)良率達(dá)94%。 結(jié)論新鮮且手法復(fù)位失敗的兒童孟氏骨折骨折,盡早復(fù)位尺骨并固定,修復(fù)環(huán)狀韌帶。陳舊性兒童孟氏骨折,則采用尺骨截骨矯形固定,環(huán)狀韌帶重建的方法復(fù)位固定橈骨小頭,以便使肘關(guān)節(jié)活動獲得良好的功能恢復(fù)。
[Abstract]:Objective to investigate the surgical treatment and clinical effect of children with Monteggia fracture. Methods from March 2003 to March 2011, 20 cases of Mencius fracture in children were followed up and analyzed. The fresh children who had failed manual reduction were treated with repair of annular ligament with ulnar reduction and internal fixation. Old children with Monteggia fracture were treated with ulnar osteotomy and annular ligament reconstruction. X-ray examination was performed regularly after operation to observe fracture healing and to check the recovery of forearm function. The recovery of forearm function was evaluated by Mackay clinical function evaluation table. Results 18 cases were followed up, 2 cases were lost, all fractures healed. According to Mackay score, 8 cases were excellent, 9 cases were good, 1 case was poor, and the excellent and good rate was 94%. Conclusion in children with fresh and failed manipulative reduction, the ulnar bone should be reduced and fixed as soon as possible to repair the annular ligament. The old Monteggia fracture in children was fixed by ulnar osteotomy and annular ligament reconstruction in order to restore the function of elbow joint.
