【摘要】：目的探討體外過(guò)敏原、呼出氣一氧化氮(FeNO)聯(lián)合檢測(cè)對(duì)識(shí)別反復(fù)喘息患兒中哮喘高�；純旱呐R床價(jià)值。方法選取反復(fù)喘息患兒148例(0.5~6歲)為研究對(duì)象,80例門(mén)診健康體檢兒童為對(duì)照組。采用Pharmacia UniCAP免疫檢測(cè)分析儀測(cè)定特異性免疫球蛋白(sIgE),納庫(kù)侖一氧化氮分析儀檢測(cè)FeNO,并評(píng)估哮喘預(yù)測(cè)指數(shù)(API)。結(jié)果 148例反復(fù)喘息患兒中,sIgE陽(yáng)性102例,陽(yáng)性率為68.9%,明顯高于對(duì)照組(11.3%,9/80)(P0.05)。喘息組患兒FeNO檢測(cè)值及陽(yáng)性率均明顯高于對(duì)照組(P0.05)。喘息患兒的API總陽(yáng)性率為32.4%,而API陽(yáng)性患兒FeNO檢測(cè)值為51±6 ppb,明顯高于API陰性患兒(13±5 ppb)(P0.05)。sIgE陽(yáng)性患兒中API陽(yáng)性檢出率為40.2%(41/102),FeNO陽(yáng)性患兒中API陽(yáng)性檢出率為50.1%(38/73),二者陽(yáng)性率比較差異無(wú)統(tǒng)計(jì)學(xué)意義。sIgE陽(yáng)性+FeNO陽(yáng)性組患兒中API陽(yáng)性檢出率為81.4%,明顯高于sIgE、FeNO二者單獨(dú)陽(yáng)性組(P0.05)。結(jié)論 FeNO、體外過(guò)敏原聯(lián)合檢測(cè)對(duì)反復(fù)喘息患兒中哮喘高危患兒的檢出率明顯優(yōu)于二者單獨(dú)檢測(cè),二者聯(lián)合檢測(cè)為兒童哮喘的早期識(shí)別診斷、干預(yù)提供了良好的手段。
[Abstract]:Objective to investigate the clinical value of the detection of extracorporeal allergen and exhaled nitric oxide (FeNO) in the identification of asthmatic high risk children with recurrent wheezing. Methods 148 children with recurrent wheezing (0.5 ~ 6 years old) were selected as control group. Pharmacia UniCAP immunoassay was used to detect specific immunoglobulin (sIgE),) and nitric oxide (no) analyzer to detect Feno, and to evaluate the asthma prediction index (API). Results among 148 children with recurrent wheezing, 102 cases were positive for sIgE, the positive rate was 68.9%, which was significantly higher than that in the control group (11.3 / 80) (P0.05). The FeNO detection value and positive rate in asthmatic group were significantly higher than those in control group (P0.05). The total positive rate of API in asthmatic children was 32.4%, while that of API positive children was 51 鹵6 ppb, which was significantly higher than that of API negative children (13 鹵5 ppb) (P0.05). The positive rate of API was 40.2% (41 / 102). The positive rate of API was 50.1% (38 / 73) in API positive children. The positive rate of API was 81.4 in the FeNO positive group, which was significantly higher than that in the Siga FeNO positive group alone (P0.05). Conclusion the detection rate of Feno and extracorporeal allergen in asthmatic children with high risk of asthma is better than that of both alone. The combined detection of Feno and extracorporeal allergen provides a good method for the early diagnosis and intervention of asthma in children.
