【摘要】：目的:總結(jié)評(píng)價(jià)精準(zhǔn)肝切除對(duì)兒童肝臟腫瘤手術(shù)治療的運(yùn)用效果。方法:回顧重慶醫(yī)科大學(xué)附屬兒童醫(yī)院肝膽外科2009年1月至2016年12月住院的78例進(jìn)行精準(zhǔn)肝切除治療的兒童肝臟腫瘤患者,包括男43例,女35例,平均年齡2歲2月。所有患兒術(shù)前均完成肝功及高分辨影像學(xué)檢查術(shù)前評(píng)估。在精確的術(shù)前評(píng)估基礎(chǔ)上進(jìn)行縝密的手術(shù)方案設(shè)計(jì)與規(guī)劃,以最小創(chuàng)傷方式離斷肝實(shí)質(zhì)并著重控制術(shù)中術(shù)后并發(fā)癥。結(jié)果:78例患兒手術(shù)過程順利,手術(shù)時(shí)間平均為199min(60min-480min),術(shù)中失血量平均412ml(10ml-3600ml),手術(shù)當(dāng)中所有患者血流動(dòng)力學(xué)指標(biāo)穩(wěn)定。術(shù)后2-3d腸功能恢復(fù)后進(jìn)食,肝功能恢復(fù)時(shí)間平均6d(4d-9d),平均住院時(shí)間8d(7d-21d)。結(jié)論:精準(zhǔn)肝切除術(shù)可安全有效地完整切除肝臟腫瘤,術(shù)中術(shù)后并發(fā)癥少,隨訪效果好。
[Abstract]:Objective: to evaluate the effect of accurate hepatectomy on children with liver tumor. Methods: from January 2009 to December 2016, 78 children with liver neoplasms underwent accurate hepatectomy, including 43 males and 35 females, with an average age of 2 months, from January 2009 to December 2016 in the Department of Hepatobiliary surgery, affiliated Children's Hospital of Chongqing Medical University. All the children underwent preoperative assessment of liver function and high-resolution imaging. On the basis of accurate preoperative evaluation, the operative scheme was designed and planned carefully. The liver was cut off with minimal trauma and postoperative complications were controlled. Results the operation time was 199min (60min-480min) and the average blood loss during operation was 412ml (10ml-3600ml). The hemodynamic indexes of all the patients were stable. The average recovery time of liver function was 6 days (4d-9d) and the average hospitalization time was 8 days (7d-21d). Conclusion: accurate hepatectomy can effectively and safely remove hepatic tumors with less postoperative complications and better follow-up results.
