【摘要】：目的分析輕度胃腸炎伴嬰幼兒良性驚厥(BICE)的臨床特點(diǎn)及預(yù)后。方法對(duì)436例BICE患兒的臨床資料進(jìn)行回顧性分析,并對(duì)其中206例患兒進(jìn)行1.5~7年的預(yù)后隨訪,邀請(qǐng)部分家長完成Weiss功能缺陷量表以評(píng)價(jià)患兒遠(yuǎn)期的社會(huì)功能。結(jié)果患兒發(fā)病年齡多在13~24月齡;發(fā)病時(shí)間多在9月至次年2月;驚厥主要表現(xiàn)為全身強(qiáng)直-陣攣發(fā)作,多在發(fā)病24 h內(nèi)發(fā)作,每次持續(xù)時(shí)間多在5 min內(nèi),可有叢集性發(fā)作。206例患兒隨訪過程中,除1例仍有vN樣放電,其余腦電圖復(fù)查均恢復(fù)正常,所有患兒家屬認(rèn)為患兒智力正常,無明顯性格改變。基于部分BICE患兒家長填寫的Weiss功能缺陷量表顯示,BICE患兒的遠(yuǎn)期社會(huì)功能與年齡、性別相匹配的健康對(duì)照兒童比較差異無統(tǒng)計(jì)學(xué)意義。結(jié)論 BICE多發(fā)生于1~2歲嬰幼兒,多表現(xiàn)為短暫的全面性發(fā)作,部分可表現(xiàn)為叢集性發(fā)作,但極少進(jìn)展為癲vN,預(yù)后良好。
[Abstract]:Objective to analyze the clinical features and prognosis of mild gastroenteritis with benign convulsion (Bice) in infants. Methods Clinical data of 436 children with Bice were retrospectively analyzed and 206 of them were followed up for 1.5 ~ 7 years. Some parents were invited to complete the Weiss dysfunction scale to evaluate the long-term social function of the children. Results the age of onset was 13 ~ 24 months, the onset time was from September to February, and the convulsions were mainly generalized tonic-clonic seizures, which occurred within 24 hours and lasted within 5 min. During the follow-up of the patients with cluster seizures, except for one patient who still had vN-like discharge, all the patients returned to normal. All the family members thought that the intelligence of the children was normal, and no significant personality changes were found. Based on the Weiss functional defect scale filled out by some parents of BICE children, there was no significant difference in long-term social function and age, sex matched healthy control children. Conclusion Bice mostly occurs in children aged 1 to 2 years old, most of them are transient comprehensive seizures, some of them are cluster seizures, but very few progress to epileptic vN, and the prognosis is good.
