【摘要】：[目的]探討鋼板前置內(nèi)固定治療青年鎖骨骨折的臨床療效。[方法]回顧性分析2013年4月~2015年5月32例行切開(kāi)復(fù)位鎖骨前置鋼板內(nèi)固定術(shù)的青年鎖骨骨折病例,并與同時(shí)期34例青年鎖骨骨折上置鋼板內(nèi)固定患者作對(duì)照。術(shù)中對(duì)游離較大骨折塊以拉力螺釘固定、較小骨塊以粗絲線固定。術(shù)后4周、8周、12周、6個(gè)月及12個(gè)月進(jìn)行隨訪,比較兩組在手術(shù)時(shí)間、總出血量、并發(fā)癥、骨折愈合、內(nèi)固定、肩關(guān)節(jié)功能及鋼板對(duì)皮膚頂壓等情況。[結(jié)果]所有患者術(shù)后均獲隨訪,時(shí)間12~36月,平均15.4個(gè)月�；颊呔鶡o(wú)血管神經(jīng)損傷、切口感染壞死及血?dú)庑氐葒中g(shù)期并發(fā)癥。前置鋼板組的手術(shù)時(shí)間平均(50.2±14.5)min、總出血量平均(99.1±25.3)ml、骨折愈合時(shí)間(16.2±8.3)周、術(shù)后12個(gè)月肩關(guān)節(jié)功能Constant平均評(píng)分(87.3±4.5)分;上置鋼板組的手術(shù)時(shí)間平均(47.6±16.5)min、總出血量平均(110.0±27.1)ml、骨折愈合時(shí)間(15.0±5.5)周、術(shù)后12月肩關(guān)節(jié)功能Constant評(píng)分平均(89.8±6.5)分,兩組上述指標(biāo)比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。上置鋼板組1例術(shù)后2個(gè)月出現(xiàn)骨折輕度上下移位,內(nèi)固定部分松動(dòng),局部制動(dòng)后術(shù)后6個(gè)月輕度畸形愈合,3例女性患者術(shù)后1個(gè)月起訴鋼板頂壓皮膚突出引起生理及心理不適。前置鋼板組1例延遲愈合,相對(duì)制動(dòng)后術(shù)后10個(gè)月愈合,無(wú)鋼板頂壓皮膚導(dǎo)致的生理及心理不適患者。[結(jié)論]前置鋼板內(nèi)固定治療青年鎖骨骨折在獲得滿意臨床療效的同時(shí)能有效減少鋼板頂壓皮膚導(dǎo)致的不適及促進(jìn)美觀,是治療青年鎖骨骨折的一種良好選擇方法。
[Abstract]:Objective: to investigate the clinical effect of anterior internal fixation with steel plate in the treatment of clavicular fracture in young people. [methods] 32 cases of young clavicle fracture treated with open reduction and anterior clavicle plate internal fixation from April 2013 to May 2015 were analyzed retrospectively and compared with 34 cases of young patients with clavicular fracture treated with upper plate internal fixation in the same period. The larger fractures were fixed with lag screws, and the smaller ones were fixed with coarse filaments. Four weeks, eight weeks, twelve weeks, six months and twelve months were followed up. The operation time, total bleeding volume, complications, fracture healing, internal fixation, shoulder joint function and skin top pressure were compared between the two groups. [results] all patients were followed up for 12 ~ 36 months (mean 15.4 months). There were no vascular and nerve injury, wound infection and necrosis, hemopneumothorax and other perioperative complications. The mean operative time was (50.2 鹵14.5) min, the mean blood loss was (99.1 鹵25.3) ml, the fracture healing time was (16.2 鹵8.3) weeks, the mean constant score of shoulder joint function was (87.3 鹵4.5) in 12 months after operation, the mean operative time was (47.6 鹵16.5) min, the mean blood loss was (110.0 鹵27.1) ml, and the fracture healing time was (15.0 鹵5.5) weeks in the steel plate group, the average operative time was (47.6 鹵16.5) min, the mean blood loss was (110.0 鹵27.1) ml, and the fracture healing time was (15.0 鹵5.5) weeks. The average constant score of shoulder joint function was (89.8 鹵6.5) points in 12 months after operation, and there was no significant difference between the two groups (P0.05). In the upper plate group, 1 case had slight displacement of fracture and partial loosening of internal fixation 2 months after operation. Three female patients with mild malunion 6 months after local immobilization complained of physiological and psychological discomfort caused by skin herniation of steel plate at 1 month after operation. There was no physiological and psychological discomfort caused by top pressure of steel plate in one patient with delayed healing and 10 months after relative immobilization. [conclusion] anterior plate internal fixation in the treatment of young clavicle fracture can effectively reduce the discomfort caused by plate top compression skin and promote beauty. It is a good choice for the treatment of young clavicle fracture.
