不同喂養(yǎng)方式對極低出生體質(zhì)量兒胃腸功能影響的比較

【摘要】:目的:探討經(jīng)幽門喂養(yǎng)(TP)和經(jīng)胃管喂養(yǎng)(IG)對極低出生體質(zhì)量兒胃腸功能的影響,以制訂合理的早期腸內(nèi)營養(yǎng)(EEN)方案。方法:將60例極低出生體質(zhì)量兒隨機(jī)分為TP組和IG組,每組各30例。觀察兩組病兒呼吸暫停發(fā)生頻率、體質(zhì)量增長速度、達(dá)出生體質(zhì)量時(shí)間、喂養(yǎng)不耐受時(shí)間、達(dá)TEN時(shí)間、宮外發(fā)育遲緩(EUGR)發(fā)生率以及胃動(dòng)素、胃泌素、住院時(shí)間、喂養(yǎng)相關(guān)并發(fā)癥、壞死性小腸結(jié)腸炎和腸穿孔等指標(biāo)。結(jié)果:TP組病兒喂養(yǎng)不耐受時(shí)間和達(dá)TEN時(shí)間均明顯低于IG組(P0.05),呼吸暫停發(fā)生頻率明顯低于IG組(P0.01)。兩組病兒的體質(zhì)量增長速度、出院時(shí)EUGR發(fā)生率、并發(fā)癥壞死性小腸結(jié)腸炎和住院時(shí)間均無顯著性差異(P0.05)。兩組病兒胃動(dòng)素和胃泌素水平出生后第8天與喂養(yǎng)前比較有顯著性差異(P0.01),且胃動(dòng)素水平TP組明顯高于IG組,差異有顯著性統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論:TP可用于胃管喂養(yǎng)耐受性差或反復(fù)出現(xiàn)呼吸暫停的極低出生體質(zhì)量兒,在進(jìn)行喂養(yǎng)時(shí)應(yīng)針對不同情況制訂個(gè)體化方案。
[Abstract]:Objective: to investigate the effects of pyloric feeding (TP) and transgastric tube feeding (IG) on gastrointestinal function in very low birth weight infants in order to establish a reasonable scheme of early enteral nutrition (EEN). Methods: 60 very low birth weight infants were randomly divided into TP group and IG group with 30 cases in each group. The frequency of apnea, growth rate of body mass, time to birth weight, feeding intolerance time, time to TEN, incidence of extrauterine growth retardation (EUGR), motilin, gastrin, hospitalization time were observed. Feeding related complications, necrotizing enterocolitis and intestinal perforation. Results the feeding intolerance time and the time of reaching TEN were significantly lower and the frequency of apnea was significantly lower in the control group than in the IG group (P0.05) and the frequency of apnea was significantly lower than that in the IG group (P0.01). There was no significant difference in the growth rate of body mass, the incidence of EUGR, the complications of necrotizing enterocolitis and hospital stay between the two groups (P0.05). There was significant difference in motilin and gastrin levels between the two groups on the 8th day after birth (P0.01), and the level of motilin in TP group was significantly higher than that in IG group (P0.01). Conclusion\% TP can be used to feed very low birth weight infants with poor tolerance or recurrent apnea in gastric tube feeding. Individualized feeding should be made according to different conditions.
