pH值、乳酸、神經(jīng)烯醇化酶及泛素羧基末端水解酶L1在早產(chǎn)兒腦損傷中的意義

【摘要】:目的探討動脈血氣pH值、乳酸、神經(jīng)烯醇化酶(NSE)及泛素羧基末端水解酶L1(UCH-L1)水平在早產(chǎn)兒腦損傷中的意義。方法選擇符合早產(chǎn)兒腦損傷診斷標(biāo)準(zhǔn)的45例作為觀察組,50例無腦損傷早產(chǎn)兒為對照組,所有病例均在生后第1、7天取動脈血查血氣分析,并留取靜脈血標(biāo)本檢測NSE和UCH-L1。結(jié)果生后第1天和第7天兩組間pH值的異常率差異有統(tǒng)計學(xué)意義(P0.05),觀察組第1天與第7天pH值異常比例比較,差異有統(tǒng)計學(xué)意義(P0.05),對照組第1天與第7天pH值異常比例比較,差異無統(tǒng)計學(xué)意義(P0.05)。兩組第1天乳酸值比較差異有統(tǒng)計學(xué)意義(P0.05),兩組第7天乳酸值比較差異無統(tǒng)計學(xué)意義(P0.05),觀察組第1天和第7天乳酸值比較,差異有統(tǒng)計學(xué)意義(P0.01)。生后第1天和第7天兩組間NSE和UCH-L1差異有統(tǒng)計學(xué)意義(P0.01),觀察組第1天NSE、UCH-L1和第7天比較,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論檢測動脈血氣、NSE及UCH-L1能夠反映早產(chǎn)兒腦損傷,為早期干預(yù)提供理論依據(jù)。
[Abstract]:Objective to investigate the significance of arterial blood gas pH, lactic acid, neuroenolase (NSE) and ubiquitin carboxyl terminal hydrolase L1 (UCH-L1) in preterm infants with brain injury. Methods 45 preterm infants who met the diagnostic criteria of brain injury were selected as the control group (n = 50). The arterial blood samples were collected on the 1st day after birth for blood gas analysis, and venous blood samples were taken to detect NSE and UCH-L1. Results there was significant difference in the abnormal pH rate between the two groups on the 1st and 7th day after birth (P0.05), and there was a significant difference between the observation group on the 1st day and the 7th day (P0.05), and on the 1st day and the 7th day in the control group, the ratio of the abnormal pH value on the first day and the seventh day was higher than that on the control group. The difference was not statistically significant (P0.05). The lactate value of the two groups on the first day was significantly different (P0.05), the lactate value of the two groups on the 7th day had no statistical significance (P0.05), the observation group on the first day and the seventh day lactic acid value comparison, the difference was statistically significant (P0.01). There was significant difference in NSE and UCH-L1 between the two groups on the 1st and 7th day after birth (P0.01), and there was significant difference between the observation group on the 1st day and the 7th day (P0.05). Conclusion NSE and UCH-L1 can reflect brain injury in premature infants and provide theoretical basis for early intervention.
