【摘要】：目的:分析影響嬰幼兒先心病術(shù)后早期撤機的術(shù)前相關(guān)因素。方法:對我院2014年1月至2017年1月在心兒科CICU監(jiān)護的先心病術(shù)后患兒作為研究對象,回顧性收集整理并分析術(shù)前相關(guān)影響因素與術(shù)后早期撤機的關(guān)系。劃分機械通氣時間≤24 h為早期撤機組,24 h為晚期撤機組。結(jié)果:(1)單因素分析結(jié)果提示早期撤機率與以下術(shù)前影響因素有關(guān):心功能Ⅰ級、NNIS分級0~1級、ASA分級1級、無肺部感染及無或輕度肺動脈高壓(均P0.05)。(2)非條件Logsitic回歸分析提示術(shù)前心功能Ⅰ級是獨立影響因素,其OR值(95%CI)為3.9(1.9~7.7)。結(jié)論:先心病嬰幼兒術(shù)前心功能Ⅰ級、無肺部感染、無或輕度肺動脈高壓、NNIS分級0~1級及ASA分級1級有利于術(shù)后早期撤機,其術(shù)前心功能Ⅰ級是有利于早期撤機的獨立影響因素。
