骨代謝相關(guān)指標在小兒原發(fā)性腎病綜合征中的表現(xiàn)

【摘要】:目的探討初診原發(fā)性腎病綜合征(PNS)患兒的骨代謝變化。方法選擇2013年2月至2016年7月初次診斷為PNS的45例患兒作為PNS組,將同期30例健康體檢兒童納入對照組;測定并比較兩組血清鈣、磷、骨堿性磷酸酶(BALP)、骨鈣素(BGP)、I型膠原交聯(lián)C末端肽(CTx)和血清抗酒石酸酸性磷酸酶5b(TRACP)、25-羥基維生素D[25(OH)D];采用超聲檢測骨密度(BMD)。結(jié)果 PNS組患兒的BALP、CTx和25-(OH)D均高于對照組,BGP、TRACP、血Ca和骨密度水平(Z-分數(shù))均低于對照組,差異有統(tǒng)計學意義(P0.01);兩組的血清磷水平差異無統(tǒng)計學意義(P0.05)。結(jié)論初診PNS患兒存在異常骨代謝,表現(xiàn)為鈣質(zhì)流失、骨形成減少、骨吸收增加等。
[Abstract]:Objective to investigate the changes of bone metabolism in children with primary nephrotic syndrome (PNS). Methods from February 2013 to early July 2016, 45 children with PNS were selected as PNS group, 30 healthy children were included in the control group, serum calcium and phosphorus were measured and compared between the two groups. Bone alkaline phosphatase (BALP), osteocalcin (BGP) type I collagen cross-linked C-terminal peptide (CTX) and serum tartrate-resistant acid phosphatase 5b (TRACP) 25-hydroxyvitamin D [25 (OH) D] were detected by ultrasound. Results the levels of BALP CTx and 25- (OH) D in the PNS group were higher than those in the control group (P 0.05), and the levels of serum Ca and bone mineral density (Z- fraction) were lower than those in the control group (P0.01), but there was no significant difference in serum phosphorus level between the two groups (P0.05). Conclusion there are abnormal bone metabolism in newly diagnosed children with PNS, such as loss of calcium, decrease of bone formation and increase of bone resorption.
