【摘要】：目的探討新生兒重癥監(jiān)護(hù)室早產(chǎn)兒醫(yī)院感染的發(fā)生情況及其危險因素,為醫(yī)院感染的預(yù)防控制提供依據(jù)。方法選取2014年1月-2015年12月醫(yī)院新生兒重癥監(jiān)護(hù)室收治的438例早產(chǎn)兒作為臨床研究對象,并對其臨床資料予以回顧性分析。結(jié)果新生兒重癥監(jiān)護(hù)室438例早產(chǎn)兒中發(fā)生醫(yī)院感染49例,感染率為11.19%;新生兒重癥監(jiān)護(hù)室早產(chǎn)兒醫(yī)院感染基礎(chǔ)疾病以新生兒肺炎和呼吸窘迫綜合征為主,分別占44.90%和36.73%;新生兒重癥監(jiān)護(hù)室早產(chǎn)兒醫(yī)院感染部位以呼吸道和皮膚為主,分別占40.82%和22.45%;感染患兒共檢出病原菌46株,其中革蘭陰性菌24株占52.17%,革蘭陽性菌16株,占34.78%,真菌6株占13.04%;胎齡≤32周、出生體質(zhì)量≤1 500g、機械通氣72h、胃腸外營養(yǎng)7d、留置胃管、應(yīng)用抗菌藥物、喂養(yǎng)不耐受、住院天數(shù)14d是新生兒重癥監(jiān)護(hù)室早產(chǎn)兒發(fā)生醫(yī)院感染的危險因素(P0.05),經(jīng)logistic多因素回歸分析,機械通氣72h、出生體質(zhì)量≤1 500g、胎齡32周、胃腸外營養(yǎng)是早產(chǎn)兒發(fā)生醫(yī)院感染的獨立危險因素。結(jié)論導(dǎo)致新生兒重癥監(jiān)護(hù)室早產(chǎn)兒發(fā)生醫(yī)院感染的危險因素相對較多,發(fā)生醫(yī)院感染的患兒其基礎(chǔ)疾病以新生兒肺炎和呼吸窘迫綜合征為主,肺炎克雷伯菌和金黃色葡萄球菌為引起醫(yī)院感染的主要致病菌,加強對早產(chǎn)兒醫(yī)院感染管理,合理化應(yīng)用抗菌藥物,對于降低早產(chǎn)兒醫(yī)院感染具有重要意義。
[Abstract]:Objective to investigate the incidence and risk factors of nosocomial infection of premature infants in neonatal intensive care unit (NICU), and to provide evidence for prevention and control of nosocomial infection. Methods 438 premature infants from January 2014 to December 2015 in neonatal intensive care unit of hospital were selected as clinical study objects and their clinical data were analyzed retrospectively. Results Nosocomial infection occurred in 49 cases of 438 premature infants in neonatal intensive care unit (NICU), the infection rate was 11.19%, the basic diseases of nosocomial infection in neonatal intensive care unit (NICU) were mainly neonatal pneumonia and respiratory distress syndrome. The nosocomial infection sites of neonatal intensive care unit premature infants were mainly respiratory tract and skin, accounting for 40.82% and 22.45% respectively, and 46 strains of pathogenic bacteria were detected, of which 24 strains were Gram-negative bacteria and 16 strains were Gram-positive bacteria.
