【摘要】：目的： 研究本院制劑復(fù)方白茅根顆粒治療兒童髖關(guān)節(jié)滑膜炎的臨床療效,分析其有效性、減少兒童髖關(guān)節(jié)滑膜炎的復(fù)發(fā)頻率的因素,為中西醫(yī)結(jié)合治療兒童髖關(guān)節(jié)滑膜炎提供一種臨床方案。 方法： 本研究采用前瞻性研究方法,將100例濕熱型兒童髖關(guān)節(jié)滑膜炎以臨床癥狀為步態(tài)跛行和髖關(guān)節(jié)疼痛,患兒隨機(jī)分為兩組,其中治療組A(基礎(chǔ)治療+復(fù)方白茅根顆粒組)和治療組B(基礎(chǔ)治療+復(fù)方白茅根顆粒+頭孢呋辛酯片)50例,對(duì)照組C(基礎(chǔ)治療)和對(duì)照組D(基礎(chǔ)治療+頭孢呋辛酯片)50例。通過(guò)對(duì)兩組患兒治療過(guò)程的觀察(1、2、4周)及隨訪,觀察兩組治療第1、2、4周患兒髖部疼痛、關(guān)節(jié)活動(dòng)度、ADL能力、復(fù)發(fā)率等指標(biāo),進(jìn)行B超檢查,按國(guó)家中醫(yī)病癥療效評(píng)價(jià)標(biāo)準(zhǔn)評(píng)價(jià)有效率,將以上數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)方法分析。 結(jié)果： 治療組與對(duì)照組髖部疼痛、關(guān)節(jié)活動(dòng)度、ADL能力、B超檢查結(jié)果、有效率比較有統(tǒng)計(jì)學(xué)意義(p0.05),治療組優(yōu)于對(duì)照組。觀察結(jié)束6個(gè)月后,治療組復(fù)發(fā)率2.1%,對(duì)照組復(fù)發(fā)率13.2%。治療組復(fù)發(fā)率比對(duì)照組低,但有統(tǒng)計(jì)學(xué)意義。 結(jié)論： ①?gòu)?fù)方白茅根顆粒治療濕熱型兒童髖關(guān)節(jié)滑膜炎療效明確,能早期改善髖部疼痛、關(guān)節(jié)活動(dòng)受限、減輕關(guān)節(jié)滑膜積液及腫脹。②復(fù)方白茅根顆粒治療濕熱型兒童髖關(guān)節(jié)滑膜炎,能明顯減少?gòu)?fù)發(fā)率。
[Abstract]:Objective: to study the clinical effect of compound Baimaogan granule in the treatment of hip synovitis in children, and to analyze its effectiveness and reduce the recurrence frequency of hip synovitis in children. To provide a clinical scheme for the treatment of hip synovitis in children with integrated traditional Chinese and western medicine. Methods: 100 cases of hip synovitis with clinical symptoms as gait claudication and hip joint pain were randomly divided into two groups. There were 50 cases of cefuroxime tablets in treatment group A (basic treatment group) and B group (basic treatment group), 50 cases in control group C (basic treatment) and 50 cases in control group (basic treatment group). By observing the treatment process of the two groups and following up for 4 weeks, we observed hip pain, joint activity and ADL ability, recurrence rate and so on in the first and second week of treatment. According to the national criteria for evaluation of the effectiveness of TCM symptoms, the above data were statistically analyzed. Results: compared with the control group, the treatment group was superior to the control group in terms of hip pain, joint motion and ADL ability, B ultrasonic examination, and the effective rate was significantly higher than that in the control group. Six months after observation, the recurrence rate was 2.1 in the treatment group and 13.2in the control group. The recurrence rate in the treatment group was lower than that in the control group, but there was statistical significance. Conclusion: 1 the curative effect of compound Baimaogan granule in treating children hip synovitis with damp-heat type is clear, and it can improve hip pain early and restrict joint movement. Reducing synovial effusion and swelling .2 compound Baomaogan granule in treating synovitis of hip joint in children with damp-heat type can obviously reduce the recurrence rate.
