【摘要】：目的探討二維斑點(diǎn)追蹤成像技術(shù)(2D-STI)在早期評(píng)估蒽環(huán)類(lèi)化療藥對(duì)血液系統(tǒng)惡性腫瘤疾病兒童左室心肌損害的價(jià)值。方法分析120例接受蒽環(huán)類(lèi)化療藥物治療的血液系統(tǒng)惡性腫瘤疾病兒童(病例組)及89例正常兒童(對(duì)照組)的超聲心動(dòng)圖,應(yīng)用斑點(diǎn)追蹤軟件分析左心室心尖長(zhǎng)軸切面的動(dòng)態(tài)圖像,獲取并分析左心室收縮期心肌縱向應(yīng)變及應(yīng)變率。結(jié)果病例組120例兒童心肌標(biāo)志物檢測(cè)及常規(guī)超聲心動(dòng)圖EF、FS檢查結(jié)果均正常,18節(jié)段應(yīng)變和應(yīng)變率值均小于正常對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。蒽環(huán)類(lèi)化療藥單位體表面積的累積劑量越大,心室壁節(jié)段應(yīng)變及應(yīng)變率越小,差異有統(tǒng)計(jì)學(xué)意義(P0.05);蒽環(huán)類(lèi)化療藥對(duì)心肌功能的影響與單位體表面積累積劑量正相關(guān),相關(guān)性有統(tǒng)計(jì)學(xué)意義(P0.05);SrL與化療藥物累積劑量相關(guān)性強(qiáng)于SL(P0.05)。結(jié)論2D-STI技術(shù)可早期發(fā)現(xiàn)蒽環(huán)類(lèi)化療藥物對(duì)血液系統(tǒng)惡性腫瘤患兒左心室長(zhǎng)軸心肌收縮功能異常,對(duì)臨床早期干預(yù)有指導(dǎo)價(jià)值。
[Abstract]:Objective to evaluate the value of 2D-STI in early assessment of left ventricular myocardial damage by anthracycline chemotherapy in children with hematological malignancies. Methods Echocardiography was performed in 120 children with hematological malignancies treated with anthracycline chemotherapeutic agents (case group) and 89 normal children (control group). The dynamic images of long axis section of left ventricular apex were analyzed by dot tracing software. The longitudinal strain and strain rate of left ventricular systolic myocardium were obtained and analyzed. Results Myocardial markers of 120 children and routine echocardiography EFF FS were significantly lower than those of the normal control group (P 0.05), and the strain and strain rate values of normal 18 segments were significantly lower than those of the normal control group (P < 0.05). The greater the cumulative dose per unit body surface area of anthracycline chemotherapeutic agents, the smaller the strain and strain rate of ventricular wall segment, the difference being statistically significant (P 0.05), the effect of anthracycline chemotherapeutic agents on myocardial function was positively correlated with cumulative dose per unit body surface area. The correlation between SRL and the cumulative dose of chemotherapeutic drugs was significantly higher than that of SLP 0.05. Conclusion 2D-STI technique can detect the abnormal systolic function of left ventricular long axis muscle in children with hematological malignancy, and it is valuable for early clinical intervention.
