【摘要】：目的對兒童心電圖表現(xiàn)有右束支傳導(dǎo)阻滯圖形者進(jìn)行臨床分析,為臨床診治提供參考。方法結(jié)合門診及住院各項輔助檢查等資料,對2014年在成都市第一人民醫(yī)院和四川大學(xué)華西第二醫(yī)院進(jìn)行心電圖檢查出現(xiàn)右束支阻滯患兒的年齡分布、病因分布及合并其他阻滯情況行統(tǒng)計分析。結(jié)果兒童不完全右束支阻滯和完全右束支阻滯在年齡分布上差異有統(tǒng)計學(xué)意義(P0.05)。兒童不完全右束支阻滯病例來源主要為先天性房間隔缺損患兒(38.85%),其次為先天性室間隔缺損經(jīng)介入封堵術(shù)后患兒(30.77%);完全右束支阻滯病例來源主要為室間隔缺損封堵術(shù)后患兒(占42.11%)。不完全右束支阻滯和完全右束支阻滯主要以單純束支阻滯圖形為主(占73.15%),其次為合并左前分支阻滯(占22.15%)。結(jié)論正常兒童心電圖可出現(xiàn)不完全右束支傳導(dǎo)阻滯,但若出現(xiàn)完全右束支阻滯多合并心臟或肺部疾患,應(yīng)結(jié)合其他臨床輔助檢查結(jié)果進(jìn)一步診治。
[Abstract]:Objective to analyze children with right bundle branch block pattern in electrocardiogram (ECG), and to provide reference for clinical diagnosis and treatment. Methods the age distribution of children with right bundle branch block after electrocardiogram examination in the first people's Hospital of Chengdu and the second Hospital of West China of Sichuan University in 2014 were analyzed. The distribution of etiology and other complications were analyzed statistically. Results there were significant differences in age distribution between incomplete right bundle branch block and complete right bundle branch block in children (P 0.05). The main sources of incomplete right bundle branch block in children were congenital atrial septal defect (CHD) 38.85, followed by congenital ventricular septal defect after interventional closure, and complete right bundle branch block mainly came from children after ventricular septal defect closure (42.11). Incomplete right bundle branch block and complete right bundle branch block were mainly composed of simple bundle branch block pattern (73.15%), followed by left anterior branch block (22.15%). Conclusion incomplete right bundle branch block can occur in normal children's electrocardiogram, but if complete right bundle branch block is more complicated with heart or lung diseases, it should be further diagnosed and treated with other clinical auxiliary examination results.
