【摘要】：目的探討甲潑尼龍輔助治療兒童難治性肺炎支原體肺炎(Refractory Mycoplasma pneumoniae pneumonia,RMPP)的療效及安全性,以指導(dǎo)臨床用藥。方法分別檢索Pubmed、Ovid、ELSEVIER ScienceDirect(SDOS)數(shù)據(jù)庫(kù)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)、中國(guó)知網(wǎng)和萬(wàn)方數(shù)據(jù)庫(kù),英文全文檢索refractory mycoplasma pneumoniae pneumonia,中文全文檢索難治性肺炎支原體肺炎和甲潑尼龍,檢索的起止時(shí)間為建庫(kù)至2016年10月。依據(jù)納入及排除標(biāo)準(zhǔn)篩選文獻(xiàn)、資料,獲得甲潑尼龍治療兒童RMPP的RCT文獻(xiàn),進(jìn)行文獻(xiàn)質(zhì)量評(píng)估后用Renman5.3軟件進(jìn)行Meta分析。結(jié)果檢索到相關(guān)文獻(xiàn)904篇,最終納入29篇RCT文獻(xiàn)進(jìn)行Meta分析。Meta分析結(jié)果提示甲潑尼龍治療兒童RMPP有效,可明顯縮短住院時(shí)間(MD=-4.05(95%CI:-4.09~-3.20,P0.00001)),血清C-反應(yīng)蛋白(CRP)水平亦明顯低于對(duì)照組(MD=-23.21(95%CI:-31.63~-14.79,P0.00001)),并能明顯縮短體溫恢復(fù)時(shí)間(MD=-3.12(95%CI:-3.57~-2.67,P0.00001))、肺部Up音消失時(shí)間(MD=-3.56(95%CI:-4.40~-2.73,P0.00001))、咳嗽消失時(shí)間(MD=-3.43(95%CI:-4.27~-2.59,P0.00001)),提高臨床有效率(OR=5.11(95%CI:3.58~7.31),P0.00001)。僅有少數(shù)輕微不良反應(yīng)。結(jié)論在大環(huán)內(nèi)酯類(lèi)抗菌藥物抗感染的基礎(chǔ)上加用甲潑尼龍輔助治療兒童RMPP療效顯著,且安全性較高。
[Abstract]:Objective to evaluate the efficacy and safety of methylprednisolone in the treatment of refractory mycoplasma pneumoniae pneumonia (Refractory Mycoplasma pneumoniae) in children. Methods Pubmedus Ovidago ELSEVIER ScienceDirect SDOS database, Chinese biomedical literature database, China Zhiwang database and Wanfang database were searched respectively. The refractory mycoplasma pneumoniae pneumonia, Chinese full-text search was used to search the refractory mycoplasma pneumoniae pneumonia and methylprednisolone respectively. The start and end of the search is until October 2016. According to the inclusion and exclusion criteria, the RCT literature of methylprednisolone for children with RMPP was obtained. The literature quality was evaluated and Meta analysis was performed with Renman5.3 software. Results 904 literatures were retrieved and 29 articles of RCT were included.
