【摘要】:目的:通過杭州市區(qū)大樣本研究了解杭州0-3歲兒童食物過敏現(xiàn)狀。方法:將2010年2月至5月在杭州市區(qū)某衛(wèi)生服務(wù)中心兒童保健科進(jìn)行常規(guī)體檢的兒童依次納入研究。家長填寫問卷調(diào)查表,對(duì)所有納入兒童進(jìn)行皮膚點(diǎn)刺試驗(yàn),篩查10種食物抗原提取物,包括雞蛋黃、雞蛋清、牛奶、大豆、花生、小麥、魚(海魚)、蝦(海蝦)、橘子和胡蘿卜。根據(jù)皮膚點(diǎn)刺試驗(yàn)和病史結(jié)果確定的疑似食物過敏病例進(jìn)行食物剔除試驗(yàn)和開放性食物激發(fā)試驗(yàn),根據(jù)激發(fā)試驗(yàn)結(jié)果確診食物過敏。結(jié)果:548例0-3歲兒童中除12例失訪,共536例兒童完成診斷過程,其中0-1歲349例,1-2歲103例,2-3歲84例。食物激發(fā)試驗(yàn)確診食物過敏兒童26例,其中0-1歲22例,1-2歲3例,2-3歲1例。食物過敏患病率為4.85%(26/536,95%(CI3.03%-6.67%),其中0-1歲兒童食物過敏的患病率為6.30%(22/349,95%CI3.75%-8.85%);1-2歲兒童食物過敏的患病率為2.91%(3/103,95%CI0%-6.16%);2-3歲兒童食物過敏的患病率為1.19%(1/84,95%CI0%-3.51%)。最常見的食物過敏原為雞蛋清(2.80%,15/536),其次是雞蛋黃(1.12%,6/536)、牛奶(0.75%,4/536)、蝦(0.56%,3/536)、魚(0.19%,1/536)。結(jié)論:杭州市區(qū)0-3歲兒童食物過敏患病率為4.85%。最常見的食物致敏原為雞蛋清,其次是雞蛋黃、牛奶、蝦和魚。
[Abstract]:Objective: to understand the status of food allergy in children aged 0-3 in Hangzhou. Methods: from February to May, 2010, the children who underwent routine physical examination in the children's health department of a health service center in Hangzhou city were included in the study. Parents filled out questionnaires, conducted skin prick tests on all children, and screened 10 food antigenic extracts, including egg yolk, egg white, milk, soybeans, peanuts, wheat, fish (sea fish, shrimp), shrimp (prawns, oranges and carrots). According to the skin prick test and the history of suspected food allergy cases, the food exclusion test and open food provocation test were carried out, and the food allergies were diagnosed according to the results of the test. Results among the 548 children aged 0-3 years, 12 cases were lost, and 536 cases completed the diagnosis process. Among them, 349 cases were 0-1 years old, 103 cases were 1-2 years old and 84 cases were 2-3 years old. 26 children with food allergy were diagnosed by food provocation test, among them 22 cases aged 0-1 years old, 3 cases aged 1-2 years, 1 case aged 2-3 years. The prevalence rate of food allergies in children aged 0 to 1 was 6.300.223.34995, and the incidence of food allergies in children aged 1-2 was 2.91 / 10395CI0-6.1666.67. The prevalence rate of food allergies in children aged 0-1 was 1.19 / 895 CI0-3.51. The incidence of food allergies in children aged 1-2 was 1.19 / 895 CI0-3.51. The most common food allergen was egg white 2.80 / 536, followed by egg yolk 1.12 and 6 / 536, milk 0.75 / 536, shrimp 0.56 / 53, fish 0.19 / 536. Conclusion: the prevalence of food allergy in children aged 0-3 in Hangzhou is 4.85. The most common food allergens are egg whites, followed by egg yolks, milk, shrimp and fish.
