【摘要】：目的分析肥胖兒童心率變異特征與血脂代謝、脂肪細(xì)胞因子含量的相關(guān)性。方法選取2014年6月-2016年7月該院收治的肥胖兒童260例作為觀察組,另取同期在該院接受疫苗接種的健康兒童200例作為對(duì)照組。采用24 h動(dòng)態(tài)心電圖獲取兩組研究對(duì)象的心率變異性指標(biāo),測(cè)定血液血脂代謝及脂肪細(xì)胞因子含量。進(jìn)一步采用Pearson檢驗(yàn)分析肥胖兒童心率變異特征與血脂代謝、脂肪細(xì)胞因子含量的相關(guān)性。結(jié)果觀察組兒童的竇性心搏RR間期標(biāo)準(zhǔn)差(SDNN)、每5分鐘RR間期平均值的標(biāo)準(zhǔn)差(SDANN)、全部相鄰RR間期之差的均方根(RSMSSD)和每5分鐘內(nèi)RR間期標(biāo)準(zhǔn)差的平均值(SDNN Index)等心率變異性指標(biāo)值均低于對(duì)照組兒童(P0.05);觀察組兒童的外周血脂質(zhì)代謝指標(biāo)中總膽固醇(TC)、三酰甘油(TG)及低密度脂蛋白膽固醇(LDL-C)含量均高于對(duì)照組兒童,高密度脂蛋白膽固醇(HDL-C)含量低于對(duì)照組兒童(P0.05);觀察組兒童的血清脂肪細(xì)胞因子瘦素(Leptin)、脂聯(lián)素(APN)和視黃醇結(jié)合蛋白4(RBP-4)含量低于對(duì)照組兒童,白介素6(IL-6)含量高于對(duì)照組兒童(P0.05);肥胖兒童的心率變異性指標(biāo)SDNN、SDANN、RSMSSD及SDNN Index水平與脂質(zhì)代謝指標(biāo)及脂肪細(xì)胞因子含量均存在直接相關(guān)性(P0.05)。結(jié)論肥胖兒童心率變異特征與機(jī)體脂質(zhì)代謝及脂肪細(xì)胞因子含量間存在相關(guān)性,過(guò)度肥胖是導(dǎo)致兒童心功能異常的重要原因。
[Abstract]:Objective to analyze the relationship between heart rate variability (HRV) and lipid metabolism and adipocyte factor content in obese children. Methods 260 obese children treated in our hospital from June 2014 to July 2016 were selected as observation group and 200 healthy children who were vaccinated in the same period as control group. The indexes of heart rate variability (HRV) were obtained by 24 h ambulatory electrocardiogram (ECG), and blood lipid metabolism and adipocyte factor content were measured. Furthermore, Pearson test was used to analyze the relationship between heart rate variability (HRV), lipid metabolism and adipocyte factor content in obese children. Results in the observation group, the standard deviation of RR interval of sinus beats, the standard deviation of the mean RR interval every 5 minutes, the mean square root mean square (RSMSSD) of all adjacent RR intervals, and the mean value of RR interval standard deviation in every 5 minutes were SDNN Index. etc. The values of heterosexual index were lower than those of control group (P 0.05), the contents of total cholesterol, triglyceride (TG) and low density lipoprotein cholesterol (LDL-C) in peripheral blood lipid metabolism of children in the observation group were higher than those in the control group. The content of HDL-C in HDL-C of high density lipoprotein cholesterol was lower than that of control group, and the serum levels of leptin and APN (APN) and retinol binding protein 4 (RBP-4) in serum of children in the observation group were lower than those in the control group, and the levels of serum leptin and leptin in the observation group were lower than those in the control group. The content of IL-6 was higher than that of control group (P 0.05), and the levels of heart rate variability (SDNN), SDANN (RSMSSD) and SDNN Index were directly correlated with lipid metabolism index and adipocyte factor content in obese children (P 0.05). Conclusion there is a correlation between heart rate variability and lipid metabolism and adipocyte factor content in obese children. Obesity is an important cause of abnormal heart function in children.
