【摘要】：目的：探討浙江省4個(gè)市(永康市、湖州市、寧海市、紹興市)孕產(chǎn)婦孕前BMI,孕期高齡、吸煙、飲酒等危險(xiǎn)因素,家族肥胖、高血壓、糖尿病等代謝綜合征史,孕期營(yíng)養(yǎng)情況和孕晚期血生化指標(biāo)對(duì)新生兒出生體重、新生兒臍血生化指標(biāo)的影響,了解孕期宮內(nèi)環(huán)境對(duì)新生兒的影響。 方法：選取2010年1月1日至2011年4月1日在永康市、湖州市、寧海市及紹興市婦幼保健院行產(chǎn)前檢查并于該院分娩的32-37周孕婦共1325例,對(duì)相關(guān)家庭背景、孕前BMI、孕期相關(guān)危險(xiǎn)因素、孕期膳食營(yíng)養(yǎng)情況、分娩情況、新生兒體重及孕晚期血液及臍血生化指標(biāo)等數(shù)據(jù)進(jìn)行整理及統(tǒng)計(jì)分析。 結(jié)果：1325例孕產(chǎn)婦每日攝入的碳水化合物(166.0±61.4)g及蛋白質(zhì)(52.7±22.2)g明顯低于推薦量(P0.01)；每日攝入的脂肪(41.0±24.2)g高于推薦攝入量(P0.05)。新生兒體重、臍血ApoAl、C肽以及胰島素水平與母孕前BMI正相關(guān)。臍血脂蛋白a高于平均值的孕母脂肪攝入量高(P0.05)；臍血胰島素及C肽高于平均值的孕母能量及碳水化合物攝入量高(P0.01)。高出生體重組母血甘油三酯水平、胰島素水平及臍血胰島素水平高于低出生體重組及正常出生體重組(P0.05)；低出生體重組臍血ApoAl與HDL水平低于高出生體重組及正常出生體重組(P0.01)。 結(jié)論：浙江省4個(gè)市孕產(chǎn)婦孕期膳食營(yíng)養(yǎng)不平衡；出生體重與孕前BMI、孕晚期母血甘油三酯、胰島素水平正相關(guān)；低出生體重兒臍血HDL水平偏低,高出生體重兒臍血胰島素水平偏高。
[Abstract]:Objective: to study the history of metabolic syndrome of pregnant women in 4 cities (Yongkang, Huzhou, Ninghai, Shaoxing) in Zhejiang province, such as pregnancy, old age during pregnancy, smoking, drinking, family obesity, hypertension, diabetes, etc. Effects of nutritional status during pregnancy and biochemical indexes of blood in late pregnancy on birth weight and umbilical blood biochemical indexes of newborns, and to understand the influence of intrauterine environment during pregnancy on newborns. Methods: from January 1, 2010 to April 1, 2011, 1325 pregnant women in Yongkang, Huzhou, Ninghai and Shaoxing maternal and child health hospitals were selected and delivered in the hospital. The data of pre-pregnancy BMI, risk factors during pregnancy, dietary nutrition during pregnancy, delivery, weight of newborn and biochemical indexes of blood and umbilical blood in the third trimester of pregnancy were collected and analyzed statistically. Results the daily intake of carbohydrate and protein were 166.0 鹵61.4g and 52.7 鹵22.2g / day in 1: 1325 pregnant women, and the daily intake of fat was 41.0 鹵24.2g / day, which was significantly lower than that of the recommended dose (P0.05g). Neonatal weight, umbilical cord blood ApoAlN C peptide and insulin levels were positively correlated with maternal pre-pregnancy BMI. The maternal fat intake of umbilical cord blood was higher than the average of serum lipoprotein a and the maternal energy and carbohydrate intake of umbilical cord blood insulin and C-peptide were higher than that of the average maternal fat intake. The levels of triglyceride, insulin and cord blood insulin in high birth weight group were higher than those in low birth weight group and normal birth weight group (P 0.05), and the levels of ApoAl and HDL in low birth weight group were lower than those in high birth weight group and normal birth weight group. Conclusion: pregnant women in 4 cities of Zhejiang Province have unbalanced dietary nutrition during pregnancy, birth weight is positively correlated with pre-pregnancy BMI, serum triglyceride and insulin levels in late trimester, HDL level in umbilical cord blood of low birth weight infants is lower than that in infants with low birth weight. Cord blood insulin level was higher in high birth weight infants.
