【摘要】：目的:探討熱休克蛋白70(HSP70)對(duì)新生兒窒息的診斷價(jià)值及其與不同窒息程度新生兒行為神經(jīng)測(cè)定(NBNA評(píng)分)的關(guān)系。方法:選擇2014年1月至2016年5月本院產(chǎn)科出生的足月窒息新生兒,根據(jù)其窒息程度分為輕度窒息組和重度窒息組;另隨機(jī)選取Apgar評(píng)分正常的足月新生兒為對(duì)照組。所有入選新生兒生后立即抽取臍動(dòng)脈血,采用酶聯(lián)免疫吸附(ELISA)法檢測(cè)各組HSP70表達(dá)水平,并在生后第7、14、28天行NBNA評(píng)分,比較各組間HSP70表達(dá)水平和NBNA評(píng)分與窒息的相關(guān)性。結(jié)果:共納入窒息新生兒81例,其中輕度窒息組46例,重度窒息組35例,對(duì)照組共納入50例。各組新生兒臍血HSP70的表達(dá)水平差異有統(tǒng)計(jì)學(xué)意義(P0.05),其中輕、重度窒息組HSP70的表達(dá)水平高于對(duì)照組[(14.4±2.7)ng/ml、(17.7±2.6)ng/ml比(11.9±2.3)ng/ml,P0.05],且重度窒息組高于輕度窒息組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。各組新生兒于生后第7、14、28天行NBNA評(píng)分比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。相關(guān)性分析顯示:HSP70水平與NBNA評(píng)分(第7、14、28天)呈負(fù)相關(guān)(r=㧟0.574、㧟0.493、㧟0.208,P均0.05)。新生兒窒息的曲線下面積(AUC)值為0.798(95%CI:0.722~0.874,P0.05),HSP70為12.9 ng/ml,約登指數(shù)最大,靈敏度84%,特異度68%。結(jié)論:臍血HSP70的表達(dá)水平與新生兒窒息程度和NBNA評(píng)分相關(guān),窒息程度越重,HSP70表達(dá)水平越高,NBNA評(píng)分越低。
[Abstract]:Objective: to investigate the diagnostic value of heat shock protein 70 (HSP70) in neonatal asphyxia and its relationship with neonatal behavioral nerve test (NBNA score). Methods: term asphyxia neonates were selected from January 2014 to May 2016 in our hospital and divided into mild asphyxia group and severe asphyxia group according to the degree of asphyxia and the term neonates with normal Apgar score were randomly selected as control group. Umbilical artery blood was collected from all the newborns immediately after birth. The expression of HSP70 in each group was detected by enzyme linked immunosorbent assay (Elisa). The NBNA score was evaluated on the 7th day 1428 after birth, and the correlation between HSP70 expression and NBNA score and asphyxia was compared. Results: 81 cases of asphyxia neonates were included, including 46 cases in mild asphyxia group, 35 cases in severe asphyxia group and 50 cases in control group. The expression of HSP70 in umbilical cord blood of all groups was significantly higher than that of control group (P 0.05). The expression of HSP70 in mild and severe asphyxia group was higher than that in control group [17.7 鹵2.6)ng/ml vs 11.9 鹵2.3 ng / ml P 0.05], and the difference was statistically significant in severe asphyxia group compared with mild asphyxia group (P 0.05). There was a significant difference in NBNA score between the groups on the 7th day and the 28th day after birth (P 0.05). The correlation analysis showed that there was a negative correlation between the level of NBNA 70 and the NBNA score (7 ~ 14 ~ 28 days). The area under the curve of asphyxia neonatorum was 0.798% 95% CI: 0.72222 0.874P0.05% HSP70 was 12.9 ng / ml, Yorden index was the largest, sensitivity was 84 and specificity was 68. Conclusion: the expression of HSP70 in umbilical cord blood is correlated with the degree of neonatal asphyxia and the NBNA score, and the higher the degree of asphyxia is, the higher the expression level of HSP70 is.
