【摘要】：目的收集有關(guān)隨機(jī)對(duì)照試驗(yàn)(randomized controlled trail,RCT)數(shù)據(jù)進(jìn)行Meta分析,評(píng)價(jià)心肺轉(zhuǎn)流(CPB)下心臟手術(shù)患兒嚴(yán)格控制血糖與常規(guī)控制血糖對(duì)圍術(shù)期相關(guān)并發(fā)癥發(fā)生率的影響,探討CPB下心臟手術(shù)患兒圍術(shù)期嚴(yán)格控制血糖可能的收益及風(fēng)險(xiǎn)。方法檢索Pubmed、Embase、OVID、WOS、Cochrane library、CBM、CNKI、萬(wàn)方及維普數(shù)據(jù)庫(kù),收集關(guān)于比較CPB下心臟手術(shù)患兒圍術(shù)期嚴(yán)格控制血糖與常規(guī)控制血糖的RCT文獻(xiàn),提取相關(guān)數(shù)據(jù),采用RevMan 5.2軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果共納入5篇RCT文獻(xiàn),總計(jì)3 372例患兒。Meta分析結(jié)果顯示,圍術(shù)期嚴(yán)格控制血糖較常規(guī)控制血糖患兒術(shù)后感染發(fā)生率明顯降低(RR=0.69,95%CI0.49~0.98),低血糖發(fā)生率明顯增高(RR=3.34,95%CI1.33~8.34),但不會(huì)降低患兒術(shù)后院內(nèi)死亡率(RR=0.78,95%CI0.47~1.30)。結(jié)論與常規(guī)控制血糖相比,心肺轉(zhuǎn)流下心臟手術(shù)患兒圍術(shù)期嚴(yán)格控制血糖能降低術(shù)后感染發(fā)生率,對(duì)術(shù)后院內(nèi)死亡率無(wú)明顯影響,但可能導(dǎo)致術(shù)后低血糖發(fā)生率增加。
[Abstract]:Objective to collect random controlled trailage data from randomized controlled trials for Meta analysis, and to evaluate the effect of strict blood glucose control and routine blood glucose control on the incidence of perioperative complications in children undergoing cardiopulmonary bypass (CPB).To explore the possible benefits and risks of strict blood glucose control in children undergoing cardiac surgery under CPB.Methods We searched the database of Pubmedus Embase OVIDB WOS Cochrane libraryCBI, Wanfang and Weip, collected and analyzed the data of RCT about strict blood glucose control and routine blood glucose control in children undergoing cardiac surgery under CPB. The data were analyzed by RevMan 5.2 software.Results A total of 5 articles of RCT were included.The incidence of infection during perioperative period was significantly lower than that in children with routine blood glucose control. The incidence of hypoglycemia was significantly higher than that of routine blood glucose control. The incidence of hypoglycemia was 3.3495 CI 1.338.34, but it did not decrease the mortality rate of RRN 0.7895 CI 0.471.30%.Conclusion compared with conventional blood glucose control, perioperative control of blood glucose in children undergoing cardiopulmonary bypass surgery can reduce the incidence of postoperative infection, and have no significant effect on postoperative hospital mortality, but may lead to an increase in the incidence of hypoglycemia after cardiopulmonary bypass.
