【摘要】：目的探討肉眼血尿對尿液多項化驗檢查結果的影響。方法回顧分析比較2014年1月至2015年12月間收治的80例有明顯肉眼血尿的IgA腎病(IgAN)患兒、40例急性鏈球菌感染后腎小球腎炎(APSGN)患兒,分別在肉眼血尿期間、肉眼血尿消失后送檢尿常規(guī)、尿蛋白定量、尿白蛋白定量、尿蛋白/肌酐、尿鈣定量、尿鈣/肌酐、腎早期損傷指標和尿蛋白電泳的變化情況。結果 IgAN和APSGN兩組患兒在肉眼血尿期間的尿蛋白定量、尿蛋白/肌酐、尿鈣定量、尿鈣/肌酐和尿小分子蛋白百分比水平較肉眼血尿消失后明顯升高,差異均有統(tǒng)計學意義(P均0.05);而尿白蛋白定量、腎早期損傷各指標(如微量白蛋白、α1微球蛋白水平、N-乙酰-β-D-葡萄糖苷酶)、血鈣/肌酐、尿白蛋白百分比水平與肉眼血尿消失后比較,差異均無統(tǒng)計學意義(P均0.05)。肉眼血尿期間,IgAN和APSGN兩組患兒的尿蛋白定量水平均高于尿白蛋白定量水平(P0.001);而肉眼血尿消失后,兩組患兒的尿蛋白定量與尿白蛋白定量水平之間的差異均無統(tǒng)計學意義(P0.05)。結論肉眼血尿時尿蛋白、尿鈣定量結果均偏高,而尿白蛋白定量則不受影響。
[Abstract]:Objective to investigate the effect of naked hematuria on the results of multiple tests in urine.Methods from January 2014 to December 2015, 80 cases of IgA nephropathy with obvious gross hematuria and 40 cases of acute streptococcal glomerulonephritis were retrospectively analyzed and compared during the period of gross hematuria.Urine routine examination, urinary protein quantification, urinary albumin quantification, urinary protein / creatinine, urinary calcium quantitative, urinary calcium / creatinine, early renal injury index and urinary protein electrophoresis after the disappearance of naked hematuria.Results the urinary protein, urine protein / creatinine, urine calcium, urine calcium / creatinine and urinary small molecule protein percentage in IgAN and APSGN group were significantly higher than those in naked eye hematuria group.The differences were statistically significant (P < 0.05), while urinary albumin quantification, early renal injury indexes (such as microalbumin, 偽 1-microglobulin level) N-acetyl- 尾 -Dglucosidase, serum calcium / creatinine,There was no significant difference between urinary albumin percentage and naked hematuria after disappearance (P < 0.05).The levels of urinary protein in the two groups were higher than those in the control group (P 0.001) during the period of gross hematuria, but there was no significant difference between the two groups after the disappearance of gross hematuria (P 0.05).Conclusion the quantitative results of urinary protein and calcium in naked hematuria were higher than those of urinary albumin.
